Information received RUVALCABA <br />a contact for Superior Truck Lines, Mario (209) 595-4693 he provided contact for <br />the owner (209) 862-9430. <br />Called and spoke with Frank Amara, owner of trucking company. After discussion <br />with Frank and with Program Coordinator it was agreed that Frank would have the <br />topsoil that still had visible milk removed and disposed. Program Coordinator <br />asked for the responsible party to be the person who caused the accident. <br />Working to get CHP report to identify the responsible party. <br />Spoke to Alex Steele with PG&E over the phone. He stated that David Larsen is <br />out for the day and will be performing the cleanup for the transformer either on <br />08/03/2018 or 08/04/2018. <br />Joanne Olivera, insurance adjuster (209) 604-0992 <br />08/01/2018 4:00 AM - Received complaint call regarding PG&E 35 gallon transformer <br />vegetable oil spill on Lone Tree Rd. at Wagner Rd. in Manteca on an orchard due <br />to a big rig truck hitting a pole mounted transformer. Called and spoke to Jennifer <br />Hope of PG&E (425-244-8236) who stated the transformer oil impacted soil only. <br />Jennifer stated the big rig went into a creek, but was not aware if any vehicle fluids <br />spilled into the creek. Jennifer stated that Dave Larsen of PG&E began clean up of <br />impacted soil from transformer oil spill, but did not finish the clean-up. A sample of <br />the oil spilled will be analyzed by a lab in Atwater. Alex Steel of PG&E is <br />overseeing Dave Larsen's clean-up of the spill. <br />I called CHP dispatch (943-8600) to verify if the big rig truck entered a creek and if <br />any vehicle fluids impacted soil or waterways. CHP dispatch stated that the big rig <br />entered a ditch, not a creek containing water. According to the CHP report no <br />vehicle fluids were released onto soil or into a waterway. <br />This complaint was obtained earlier in the day by Muniappa Naidu for the big rig <br />truck's spillage of 5,000 gallons of milk into the orchard. Muniappa Naidu stated <br />that the person on C (Cesar Ruvalcaba) will be assigned this complaint, since I <br />was responding to another after-hours spill. <br />FLORIDO <br />08/01/2018 Completed narrative for after hours phone response and discussion with Cesar FLORIDO <br />Ruvalcaba regarding the complaint. <br />5105.rpt Page 1 of 2
