STATE OF CALIFORNIA
NARRATIVE/SUPPLEMENTAL PAGE 110F 13

NCIC NUMBER OFFICER I.D. NUMBER
9265 018767 9265-2018-02430
DATE OF INCIDENT TIME
07/31/2018 1328
1 OPINIONS AND CONCLUSIONS
2
3 SUMMARY
4 Party 1 (P-1) (Lindsey) was driving V-1 (Chevrolet) northbound on a dirt access road to an
5 almond orchard and stopped for the stop at Lone Tree Rd. waiting for traffic to clear. P-1 was
6 wearing his lap and shoulder safety restraint system.
7
8 Party 2 (P-2) (Flores) was driving V-2 westbound on Lone Tree Rd., east of Wagner Rd. at 55
9 MPH. P-2 was wearing his lap and shoulder safety restraint system.
10
11 P-1 drove V-1 from the dirt access road onto Lone Tree Rd., to proceed westbound and turned
12 directly in the path of V-1. P-2 turned V-2 left to avoid a collision but was unable to in time and the
13 front of V-2 struck the rear of V-1's trailer. V-2 left the roadway to the south and struck a wooden
14 utility pole. V-2 continued into an almond orchard and struck several small almond trees. P-2
15 turned V-2 back to the right towards the road where it overturned onto its right side, across both
16 lanes. V-1's trailer became detached and came to rest on its side in the eastbound lane. V-1
17 rotated clockwise and traveled in a northerly direction, ran off the road and down a canal
18 embankment into the water.
19
20 After the collision, V-1 came to rest on its wheels, facing an easterly direction and partially
21 submerged in the canal north of the roadway and east of V-2. V-2 came to rest on its right side
22 facing a northerly direction and blocking both lanes and partially blocking both dirt shulders.
23
24 AREAS OF IMPACT (A01)
25 The A01 was determined by using the station line.
26 A01#1 (V-1 vs V-2) was located at station 2+91 and 6 feet left.
27 A01#2 (V-1 trailer overturn) was located at station 2+65 and 3 feet left.
28 A01#3 (V-2 vs utility pole) was located at station 2+06 and 3 feet right.
PREPARED BY
ZACHARY J YOSH
I.D.
