NUMBER <br />07/31/2018 1328 9265 018767 9265-2018-02430 <br />1 Utility Pole <br />2 A utility pole was detached from its base and destroyed as a result of the collision, it is located and <br />3 labeled on the factual diagram. <br />4 <br />5 STATEMENTS <br />6 <br />7 Party 1 (P-1) (Lindsey) was contacted at the scene and related the following: He was driving <br />8 north in a dirt driveway and stopped at Lone Tree Rd. He observed V-2 east of his location on <br />9 Lone Tree Rd. and driving westbound towards him. He believed he had enough room to pull onto <br />10 Lone Tree Rd. safely and began a left turn into the westbound lane of Lone Tree Rd. P-1 felt an <br />11 impact to the rear of V-1 and lost control of V-1. V-1 traveled in a northerly direction off the road <br />12 and into a canal. After the collision, P-1 exited V-1 and waited for emergency personnel to arrive. <br />13 <br />14 Party 2 (P-2) (Flores) was interviewed at the scene and related the following: he was driving V-2 <br />15 westbound on Lone Tree Rd., east of Wagner Rd. at 55 MPH. V-1 pulled from a dirt access road <br />16 on the south side of Lone Tree Rd., onto westbound lane of Lone Tree Rd. and directly in the path <br />17 of V-2. P-2 turned V-2 to the left to avoid a collision and lost control.
