Complaint Investigation Summary Report #: 5105 <br />COMPLAINT ID: COTPGGVF3 Site Location: 233 N. ACACIA AVE. <br />Activity Summary <br />Activity Date Recorded by <br />08/15/2020 6:30am-12:00pm GAITAN <br />Received a pager notification from SJC GH. Informed me of transformer oil spill at <br />233 Acacia Ave. Manteca, CA Gave me contact information. Received call from <br />AJ Doudna, PGE. Informed me that pole mounted transformer released <br />20-gallons of mineral oil on to backyard of private property and alleyway located in <br />the back of the house. No waterways were impacted. He stated that he was <br />unsure if area has been contained. He stated that PARC Environmental will be on <br />site around 0930 am to begin clean up. Contacted Haza Saeed ILead Senior <br />RENS' based on discussion, I left to go inspect the area. <br />Knocked on private residence in 233 Acacia Ave Apt. A, and 229 Acacia Ave. apt. <br />A there was no answer for both properties. Left my business card on the door. <br />Checked alleyway and observed caution tape around pole mounted transformer <br />and observed through the broken fence of the backyard of private residence (233 <br />Acacia Ave Apt. A) was caution taped. Generated notice to abate and sent to Alex <br />Steele, PGE. <br />Contacted AJ Doudna, PGE. Asked if PARC Environmental was arriving any time <br />soon. He stated that he has not received any notice that they were arriving late. He <br />gave me the contact for PARC (559) 232-3427. Stated that he will contact them as <br />well. <br />Contacted PARC environmental, spoke to Patrick, he stated that his ETA was <br />10:10 am. Discussed what I observed, I let him know that! have not gone through <br />the backyard. <br />Began working on photo log. <br />Met with Patrick and discussed clean up. He stated that he could not begin <br />cleaning up the back yard of the property until property owner gave permission. He <br />stated that he would begin working on alleyway for the time being. I left a voicemail <br />for AJ. Received a call from Dave Larsen, Tool Technician, stated that he is <br />currently trying to contact Alex Steele, Field Scientist to get the property owner <br />information. Patrick stated that he was going to reach out to AJ again. I knocked <br />on the front door of 233 Acacia Ave. Apt A, and the tenant answered. He was a <br />Spanish only speaker, asked if the property owner could come, he stated that he <br />is on his way and 5 minutes out. Property owner for 233 Acacia Ave. apt A arrived, <br />Ricardo Oropeza. Ricardo discussed with myself and Patrick regarding clean up, <br />after discussion Ricardo gave Patrick permission to begin working on the <br />backyard. I asked Patrick if he got in contact with the other property owner next <br />door, he stated that he did not. Patrick stated, after review of backyard impacted <br />with transformer oil, that he was not equipped to begin doing clean up on the <br />backyard. He stated that he needed a roll-off bin. I received a call from Dave; he <br />stated that he is getting a roll-off bin out there and that he will arrive on site. I let <br />Patrick know that Dave will be on site sometime today. Patrick stated that he <br />would contact his boss regarding if the spill would be cleaned up today. I let <br />Patrick know that I was going to refuel the ER truck and will contact him regarding <br />the clean-up progress. <br />Received a call from Patrick, he stated that he spoke with Alex Steele and his <br />direct boss at PSC. He stated that they both agreed that the spill cannot be <br />cleaned up today and that they will tarp the area and get the proper equipment by <br />Monday. Contacted Haza Saeed 'Lead Senior REHSI and discussed the update <br />with him, based on discussion I contacted Patrick and let him know to keep me <br />updated on when clean up will resume. He stated that he is finishing the alleyway <br />and has generated one 55-gallon container with liquid waste transformer oil and <br />one 55-gallon drum with soil contaminated with transformer oil. I requested photos <br />of the clean up and pictures of the manifest and hazardous waste labels. He <br />stated that he would send them over. <br />Received a call from Dave Larsen, he stated that the clean-up will happen today <br />and that he will contact Patrick and make sure the clean-up will finish today.
