Complaint Investigation Summary Report #: 5105 COMPLAINT ID: COTPGGVF3 Site Location: 233 N. ACACIA AVE. Activity Summary Activity Date Recorded by 08/15/2020 6:30am-12:00pm GAITAN Received a pager notification from SJC GH. Informed me of transformer oil spill at 233 Acacia Ave. Manteca, CA Gave me contact information. Received call from AJ Doudna, PGE. Informed me that pole mounted transformer released 20-gallons of mineral oil on to backyard of private property and alleyway located in the back of the house. No waterways were impacted. He stated that he was unsure if area has been contained. He stated that PARC Environmental will be on site around 0930 am to begin clean up. Contacted Haza Saeed ILead Senior RENS' based on discussion, I left to go inspect the area. Knocked on private residence in 233 Acacia Ave Apt. A, and 229 Acacia Ave. apt. A there was no answer for both properties. Left my business card on the door. Checked alleyway and observed caution tape around pole mounted transformer and observed through the broken fence of the backyard of private residence (233 Acacia Ave Apt. A) was caution taped. Generated notice to abate and sent to Alex Steele, PGE. Contacted AJ Doudna, PGE. Asked if PARC Environmental was arriving any time soon. He stated that he has not received any notice that they were arriving late. He gave me the contact for PARC (559) 232-3427. Stated that he will contact them as well. Contacted PARC environmental, spoke to Patrick, he stated that his ETA was 10:10 am. Discussed what I observed, I let him know that! have not gone through the backyard. Began working on photo log. Met with Patrick and discussed clean up. He stated that he could not begin cleaning up the back yard of the property until property owner gave permission. He stated that he would begin working on alleyway for the time being. I left a voicemail for AJ. Received a call from Dave Larsen, Tool Technician, stated that he is currently trying to contact Alex Steele, Field Scientist to get the property owner information. Patrick stated that he was going to reach out to AJ again. I knocked on the front door of 233 Acacia Ave. Apt A, and the tenant answered. He was a Spanish only speaker, asked if the property owner could come, he stated that he is on his way and 5 minutes out. Property owner for 233 Acacia Ave. apt A arrived, Ricardo Oropeza. Ricardo discussed with myself and Patrick regarding clean up, after discussion Ricardo gave Patrick permission to begin working on the backyard. I asked Patrick if he got in contact with the other property owner next door, he stated that he did not. Patrick stated, after review of backyard impacted with transformer oil, that he was not equipped to begin doing clean up on the backyard. He stated that he needed a roll-off bin. I received a call from Dave; he stated that he is getting a roll-off bin out there and that he will arrive on site. I let Patrick know that Dave will be on site sometime today. Patrick stated that he would contact his boss regarding if the spill would be cleaned up today. I let Patrick know that I was going to refuel the ER truck and will contact him regarding the clean-up progress. Received a call from Patrick, he stated that he spoke with Alex Steele and his direct boss at PSC. He stated that they both agreed that the spill cannot be cleaned up today and that they will tarp the area and get the proper equipment by Monday. Contacted Haza Saeed 'Lead Senior REHSI and discussed the update with him, based on discussion I contacted Patrick and let him know to keep me updated on when clean up will resume. He stated that he is finishing the alleyway and has generated one 55-gallon container with liquid waste transformer oil and one 55-gallon drum with soil contaminated with transformer oil. I requested photos of the clean up and pictures of the manifest and hazardous waste labels. He stated that he would send them over. Received a call from Dave Larsen, he stated that the clean-up will happen today and that he will contact Patrick and make sure the clean-up will finish today.
