The following is an itemized list of hazardous materials violations that have not been <br /> addressed for Harney Lane 76 as of June 23 , 2021 . <br /> Open violations from May 04, 2021 inspection <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date) : <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation : <br /> Violation #11 - Failed to provide or document initial and/or refresher training to appropriate personnel . <br /> OBSERVATION : When employee Dhury Cheuds was asked regarding the facility's response plan , Dhury stated he <br /> started 1 month ago and has not received training . Per manager Jesse Singh , the hazardous material business <br /> plan training has not been provided to employees at this facility. <br /> REGULATION GUIDANCE : The business plan shall include provisions for ensuring that appropriate personnel <br /> receive initial and annual refresher training . All employees shall be trained in safety procedures in the event of a <br /> release or threatened release of a hazardous material , including , but not limited to , familiarity with the following <br /> plans and procedures : <br /> (A) Immediate notification contacts to the appropriate local emergency response personnel and to the unified <br /> program agency . <br /> ( B) Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons , property, or the environment. <br /> (C) Evacuation plans and procedures , including immediate notice , for the business site . <br /> This training shall be documented electronically or by hard copy and shall be made available for a minimum of three <br /> years . <br /> CORRECTIVE ACTION : Immediately provide employee training for appropriate personnel and submit a copy of the <br /> training records to the EHD . <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date) : <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation : <br /> Violation #12 - Failed to have the HMBP readily available to facility or EHD personnel . <br /> OBSERVATION : Facility could not provide the Emergency Response Plan and the inventory information upon <br /> request. The manager and employees on site did not have access to the Emergency Response Plan and the <br /> inventory information on-site . <br /> REGULATION GUIDANCE : The emergency response plans and procedures , the inventory of information required <br /> by this article , and the site map required by this section shall be readily available to personnel of the business or the <br /> unified program facility with responsibilities for emergency response or training pursuant to this section . <br /> CORRECTIVE ACTION : Immediately make arrangements to have the business plan readily available for review. <br /> t <br /> Page 2 of 3 <br />
