Requests for a public hearing should include the following information: 1) the name, address, and telephone number of the in- dividual, organization, or other entity requesting a hear- ing; 2) a brief statement of the requesting person's interest in the proposed action and of information that the requesting person intends to submit at such hearing; and 3) the signature of the individual making !the request, or if the request is made on behalf of an organization or other entity, the signature of a responsible official of the organization or other entity. Notice of the final decision will be sent to each person who has sent or delivered written comments or who has requested notice of the final permit decision. If issued, the permit will become effective 30 days from the date of issuance un- less: 1. a later effective date is specified in the permit; 2. an administrative review is requested under 40 CFR §124. 19 ; or 3. there are no comments requesting a change to the draft permit, in which case the final decision may become ef- fective immediately upon issuance. All persons are advised that they must raise all reasonably ascertainable issues and submit all reasonably available arguments and factual grounds supporting their position by the close of the comment period, pursuant to I40 CFR §124 . 13 . In any review of the final permit deci- sion, no issues may be raised that were not submitted to the administrative record unless good cause is shown for the failure to do so, pursuant to 40 CFR §124 .76.
