Logs and Tests [ 5146 . 12(d) ] The appropriate logs and tests as specified in Part III , Section A, of this permit , shall be conducted during the drilling and construction of the well. A descriptive report interpreting the results of such logs and tests shall be prepared by a knowledge- able log analyst and submitted to the Director. 5 . Construction Changes The permittee shall give advance notice, to the Director for review, of any planned changes in the construction of the per- mitted injection well . Unless approval is given by the Director , no significant deviations from the above construction plan will be authorized. B. OPERATION During the period beginning on the effective date of this permit and ending ten years from the effective date of this permit , the permittee is authorized to inject only non-hazardous pretreated process wastewater generated by the onsite meat processing plant and extracted groundwater into the injection well under the fol- lowing conditions: 1. Injection Formation Injection shall be limited to the Domengine Formation in the sub- surface interval between approximately 2650 feet and 3150 feet. 2. Confining Laver The injection formation is confined by the Nortonville Formation which immediately overlies the Domengine Formation from ap- proximately 2375 feet to 2650 feet. 3 . Iniection Pressure Limitation [§146 . 13 (a) ] Injection pressure at the wellhead shall not exceed a maximum which shall be calculated so as to assure that the pressure in the injection zone during injection does not initiate new frac- tures or propagate existing fractures in the injection zone. In no case shall injection pressure initiate fractures in the con- fining zone or cause the movement of injection or formation fluids into an underground source of drinking water. The maximum injection pressure , measured at the wellhead, shall not exceed 810 psig. 13
