Iniection Pressure Modification <br /> The injection pressure may be modified from that listed above if <br /> a hydraulic fracture gradient test ( i . e. , step rate test or <br /> equivalent) indicates the formation fracture gradient is other <br /> than that calculated pursuant to 40 CFR §147 . 253 . <br /> 5 . Additional Injection Limitation <br /> Injection between the outermost casing protecting underground <br /> sources of drinking water and the well bore is prohibited. <br /> 6 . Iniection Rate <br /> The injection rate shall not exceed 195 gpm at any time. <br /> 7 . Annulus Measurements <br /> The annulus between the tubing and the long string casing shall <br /> be filled with treated fresh water to prevent corrosion and/or <br /> bacterial growth. A minimum surface annular pressure of 100 psig <br /> shall be maintained at all times. <br /> The permittee shall notify the Director in accordance with Part <br /> I , Section E , Item 12(d) of this permit when a change in the to- <br /> tal annular fluid volume exceeds two (2) gallons at Standard Tem- <br /> perature and Pressure (STP) . A volume adjustment in the annulus <br /> is considered a physical alteration and is subject to the report- <br /> ing requirements of Part I , Section E, Item 12(a) . The total an- <br /> nular fluid volume range and the minimum annular pressure must be <br /> maintained unless a demonstration for suitable alternate values <br /> is submitted by the permittee and has been approved, in writing , <br /> by the Director. <br /> C. MONITORING REQUIREMENTS <br /> 1 . Sampling and Analytical Methods[ §144. 51 ( j ) ( 1) and <br /> §144. 52(a) (5 ) ) <br /> Samples and measurements taken for the purpose of monitoring <br /> shall be representative of the monitored activity. The method <br /> used to obtain a representative sample of the fluid to be <br /> analyzed and the procedure for analysis of the sample are as fol- <br /> lows: <br /> (a) Grab samples shall be collected at the sampling valve <br /> at the wellhead and used for laboratory analysis for <br /> physical and chemical characteristics. <br /> (b) Temperature , annular pressure , and injection pressure <br /> shall be measured at the wellhead. <br /> 14 <br />
