PART II WELL SPECIFIC CONDITIONS FOR UIC.PERMITS A. CONSTRUCTION 1. Siting [§146. 12 (a) ] The injection well shall inject only into a formation which is beneath the lowermost formation containing, within one quarter mile of the well bore, an underground source of drinking water. 2 . Casing and Cementing [§146. 12 (b) ] Notwithstanding any other provisions of this permit, the per- mittee shall case and cement the well to prevent themovementof fluids into or above underground sources of drinking water. The casing, tubing, liner, and cement used in the construction of the well shall be designed for the life expectancy of the well. . The following specifications apply to the injection well: - Conductor pipe - 30 feet of 16 inch or larger pipe - Surface casing - 1700 feet of 10-3/4 inch, K-55 , 40 . 5 pounds/foot, ST & C casing cemented from the surface to 1700 feet. - Long string casing - 2700 feet of 7 inch, K-55 and 8 rnd, 20 pounds/foot, ST & C casing cemented from the surface to 2700 feet. - Tubing - 4-1/2 inch, J-55, 12 .75 pounds/foot, API EUE tubing from the surface to 2600 feet with a hydraulic set packer set at approximately 2650 feet. - Liner - 5-1/2 inch, K-55, 17 pounds/foot, FJ slotted liner from 2650 feet to 3150 feet. 3 . Tubing and Packer Specifications [§146. 12 (c) (2) ] Injection may only take place through 4-1/2 inch tubing with a locking, retrievable packer set within the long string casing im- mediately above the injection zone (at approximately 2700 feet) . The tubing and packer shall be maintained in a manner which is compatible with the injection operation specified in Part II, Section B, and to prevent the movement of fluids into or above underground sources of drinking water. The tubing and packer shall be corrosion-resistant and be of sufficient strength to resist wellbore pressures developed during the life of the well. 12
