- drill core and sidewall core analysis <br /> - effective thickness of available sand (net sand) <br /> - individual thicknesses of sand beds or layers that are anticipate3 to take in- <br /> jection fluids <br /> (5) physical and chemical characteristics of the formation fluids <br /> C) Confining layer information <br /> (1) formation fluid pressure <br /> (2) other physical and chemical characteristics of the confining zone <br /> matrix including: <br /> - hydraulic conductivity, vertical and horizontal <br /> - porosity <br /> - drill core and sidewall core analysis <br /> - thicknesses of significant shale layers or beds <br /> (3) physical and chemical characteristics of the formation fluids <br /> D) Maintenance and calibration of monitoring equipment <br /> Prior to commencing injection the permittee shall submit, to the Director for <br /> review, procedures to be used for the proper operation and maintenance of <br /> the injection well. These procedures shall include a schedule for calibrating <br /> equipment, updating charts, regular maintenance of the well and system, and <br /> other related maintenance and monitoring checks. The Director must be <br /> notified of any changes in the above procedures. <br /> E) Continuous monitoring devices. <br /> The permittee shall install continuous monitoring devices to monitor tem- <br /> perature; injection pressure, rate, and volume; and annular pressure and an- <br /> nular fluid volume in accordance with Part II, Section C, of this permit. <br /> The equipment shall be installed and operating prior to commencing injec- <br /> tion of waste fluid into the injection well. <br /> F) Automatic alarms and shut-off devices. <br /> The permittee shall install automatic alarms and shut-off devices which will <br /> immediately notify the operators of a malfunction in the event that the in- <br /> jection pressure at the well head exceeds the maximum injection pressure <br /> authorized by this permit. The automatic alarms shall also notify the <br /> operators of a significant change in the annular fluid volume and pressure in <br /> accordance with Part 11, Section B, item 6, of this permit. <br /> The equipment shall be installed and operating prior to the effective date of <br /> this permit. <br /> G) Plugging and abandonment plan <br /> 19 <br />
