OBSERVATION: -The designated operator failed to check and note the correct test dates on the following designated operator inspection report: -6/19/20-monitoring certification, spill container testing, overfill prevention testing -7/22/2020 to 1/21/2021-overfill prevention testing -The designated operator inspections for the following months were performed more than 30 days after the previous inspection: - Initial inspection on 6/19/2020, next inspection performed on 7/22/2020(33 days) - Initial inspection on 8/21/2020, next inspection performed on 9/25/2020(35 days) - Initial inspection on 11/18/2020, next inspection performed on 12/21/2020 (33 days) - Initial inspection on 2/17/2021, next inspection performed on 3/21/2021 (32 days) - Initial inspection on 3/21/2021, next inspection performed on 4/21/2021 (31 days) -The designated operator failed to check and note all required items on the visual inspection reports. The incorrect and missing information included the following from the inspection reports: -CERS ID number was missing from 6/19/2020 to 9/25/2020 and 2/17/2021 -Name of operator was missing from 6/19/2020 to 5/21/2021 -Spill container testing was marked as fail on 7/22/2020 DO report but passing on all others -Owner/operator did not respond to all compliance issues listed in section 3 on 9/25/2020, 11/18/2020 to 1/21/2021, 3/21/2021 -Designated operator reports were not signed within the required time frame. -The 8/21/2020 designated operator report was not signed by the owner/operator REGULATION GUIDANCE: -During the inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. -The designated operator visual inspections shall be conducted within 30 days or less of the previous inspection. -During the visual inspection,the designated operator shall check that all required testing and maintenance for the UST system have been completed and shall check all required items on the report. -Within 48 hours of the completion of the designated UST operator visual inspection,the designated operator shall sign and provide the owner or operator with a copy of the inspection report. Within 72 hours of being provided a signed copy of the report,the owner or operator shall provide a description of each corrective action taken or to be taken and shall sign and date the report, acknowledging the identified compliance issues. CORRECTIVE ACTION: -Ensure that designated operators performing inspections at this facility are including all of the required information on the reports. -Ensure that all designated operator inspections are performed within the required time frames. -Ensure all designated operator inspection reports are completed and signed within the required time frame in the future.
