OBSERVATION: The Facility Employee Training Certificate dated 9/23/2019 is missing the CERS ID number and the date of assuming responsibility as a facility employee. REGULATION GUIDANCE:All site employees must be trained prior to taking responsibility as a facility employee if after October 1, 2018, or within 30 days of hire if prior to October 1, 2018. The Facility Employee Training Certificate shall contain accurate information for all employees trained. CORRECTIVE ACTION: Update the Facility Employee Training Certificate to contain the accurate information and submit a copy of the updated form to the EHD. This is a repeat violation, Class II. 302 CCR 2630(d), 2641(a)Leak detection equipment not installed/maintained/located to detect leaks at earliest opportunity. OBSERVATION: Schrader valves were open on the clamshell, but pointed up and not positioned in a way that leaks will be detected at the earliest opportunity in all turbine sumps. REGULATION GUIDANCE: Monitoring equipment shall be maintained to be able to detect a leak at the earliest possible opportunity. CORRECTIVE ACTION: Immediately contact a properly licensed,trained, and certified contractor to ensure that any releases in the piping will be detected at the earliest opportunity. This is a Class II violation. 306 HSC 25291(e)Failure to remove, analyze, and properly dispose of water in secondary containment(pre-Jul 2003). OBSERVATION: Liquid was observed in the following: -Approximately 2 cups of a blue-green liquid was observed in 5000 gallon 91 turbine sump -Approximately 4 cups of liquid was observed in the diesel turbine sump REGULATION GUIDANCE: If water could enter into the secondary containment by precipitation or infiltration, it must be removed and disposed of properly. CORRECTIVE ACTION: Immediately remove this liquid, make a hazardous waste determination per Title 22 hazardous waste regulations, and manage it accordingly. Ensure that all sumps and annular spaces are maintained free of liquid. This is a repeat violation, Class II.
