The Facility Employee Training <br /> Certificate shall contain accurate information for all employees trained. <br /> CORRECTIVE ACTION: Update the Facility Employee Training Certificate to contain the accurate information and <br /> submit a copy of the updated form to the EHD. <br /> This is a repeat violation, Class II. <br /> 302 CCR 2630(d), 2641(a)Leak detection equipment not installed/maintained/located to detect leaks at earliest <br /> opportunity. <br /> OBSERVATION: Schrader valves were open on the clamshell, but pointed up and not positioned in a way that leaks <br /> will be detected at the earliest opportunity in all turbine sumps. <br /> REGULATION GUIDANCE: Monitoring equipment shall be maintained to be able to detect a leak at the earliest <br /> possible opportunity. <br /> CORRECTIVE ACTION: Immediately contact a properly licensed,trained, and certified contractor to ensure that any <br /> releases in the piping will be detected at the earliest opportunity. <br /> This is a Class II violation. <br /> 306 HSC 25291(e)Failure to remove, analyze, and properly dispose of water in secondary containment(pre-Jul 2003). <br /> OBSERVATION: Liquid was observed in the following: <br /> -Approximately 2 cups of a blue-green liquid was observed in 5000 gallon 91 turbine sump <br /> -Approximately 4 cups of liquid was observed in the diesel turbine sump <br /> REGULATION GUIDANCE: If water could enter into the secondary containment by precipitation or infiltration, it <br /> must be removed and disposed of properly. <br /> CORRECTIVE ACTION: Immediately remove this liquid, make a hazardous waste determination per Title 22 <br /> hazardous waste regulations, and manage it accordingly.
