i The following is an itemized list of underground storage tank violations that have not been addressed for SINCLAIR/DINO MART as of July 19, 2021. Open violations from May 26, 2021 inspection ❑ This violation was corrected This violation will be corrected by(date): 1 ❑ Supporting documents included Describe actions taken or will be taken to correct violation: 1021 Violation#210 -Failure to provide Initial training,train every 12 months, and have a trained employee present. OBSERVATION The designated operator employee training for all employees was not not current. The current training records on site were dated 2017. REGULATION GUIDANCE:The designated operator shall train facility employees in the proper operation and maintenance of the UST system at least once every 12 months. The training shall be conducted before the individual performs the duties of a facility employees.The training shall include, but is not limited to: 1. Operation of the UST system in a manner consistent with the facility's best management practices 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan 3. Employee's role with regard to spills and overfills as specified in the facility's response plan 4. Name of the contact person(s)for emergencies and monitoring equipment alarms Initial training shall also include a site-specific practical demonstration that provides both sharing information and evaluating knowledge for responding to emergencies, leak detection, monitoring alarms, spills and overfills. At least one of the facility employees present during operating hours shall have current training. CORRECTIVE ACTION: Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of the training records to the EHD. ❑ This violation was corrected Ut This violation will be corrected by (date): i ❑ Supporting documents included Describe actions taken or will be taken to correct violation: r i Violation#306-Failure to remove,analyze, and properly dispose of water in secondary containment (pre-Jul 2003). OBSERVATION Liquid was observed in the 87 main fill sump and 91 fill sump. i REGULATION GUIDANCE If water could enter into the secondary containment by precipitation or infiltration, it must be removed and disposed of properly. CORRECTIVE ACTION: The service technician removed approximately 5 gallons of liquid from the 87 main fill sump and 5 gallons of liquid from the 91 fill sump.
