The following is an itemized list of underground storage tank violations that have not been addressed for SINCLAIR/DINO MART as of October 22 , 2021 . Open violations from May 26 , 2021 inspection Violation #208 - Failure to comply with the designated operator inspection requirements . OBSERVATION : The latest designated operator report on site was dated April 1 , 2020 . The facility did not have designated operator reports for the following months : May 2020 , June 2020 , July 2020 , August, 2020 , September 2020 , October 2020 , November 2020 , December 2020 , January 2021 , February 2021 , March 2021 , April 2021 , and May 2021 . REGULATION GUIDANCE : The designated operator visual inspections shall be conducted within 30 days or less of the previous inspection . CORRECTIVE ACTION : Ensure that all designated operator inspections are performed within the required time frames . ❑ This violation was corrected ❑ This violation will be corrected by (date ) : Supporting documents included Describe actions taken or will be taken to correct violation : Violation #210 - Failure to provide initial training , train every 12 months , and have a trained employee present. OBSERVATION : The designated operator employee training for all employees was not not current. The current training records on site were dated 2017 . REGULATION GUIDANCE : The designated operator shall train facility employees in the proper operation and maintenance of the UST system at least once every 12 months . The training shall be conducted before the individual performs the duties of a facility employees . The training shall include , but is not limited to : 1 . Operation of the UST system in a manner consistent with the facility's best management practices 2 . Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan 3 . Employee 's role with regard to spills and overfills as specified in the facility' s response plan 4 . Name of the contact person (s ) for emergencies and monitoring equipment alarms Initial training shall also include a site-specific practical demonstration that provides both sharing information and evaluating knowledge for responding to emergencies , leak detection , monitoring alarms , spills and overfills . At least one of the facility employees present during operating hours shall have current training .
