Complaint Investigation Summary Report* 5105 <br /> COMPLAINT ID: CONKGITOF Site Location: 1501 N JACK TONE RD <br /> Activity Summary <br /> Activity Date Recorded by <br /> 09/24/2020 Received call from Kathy of Ripon Fire (209-236-8100) on ER back up phone at BAKER <br /> 9:05 am. She reported a 35 gallon diesel spill into a storm drain at Flying J at <br /> 1501 N. Jack Tone Road in Ripon. Jeremy Abundiz (209-996-6804) with Ripon <br /> Fire is on site. <br /> Called Jeremy Abundiz. He said that a truck backed into a semi hitting the saddle <br /> tank causing the release. He said that Ripon Police Department is there and sent <br /> me the information for the people on site. Lloyd Reed (Ranokx Logistics) was the <br /> driver of the truck that had the fuel tank punctured. Joseph Leandro <br /> (209-896-7644)was the driver with Morrill Industries that caused the accident. <br /> Called Elizabeth Quilici(City of Ripon Engineering Director) and reported incident <br /> to her at 209-599-2108. <br /> Discussed complaint with Elena Manzo (Lead Sr. REHS) and headed out to the <br /> site. Arrived on site and met with Officer Paul Rourick (209-599-2102) of Ripon <br /> Police Department and Mike Pole (209-599-2108) of City of Ripon Public Works. <br /> Spill site was located at storm drain north of the truck fuel island. Spill on <br /> pavement covered an approximate area of 15 x 40 feet. Spill area was covered <br /> with absorbent, surrounded by cones and storm drain was surrounded by <br /> absorbent socks to prevent any additional diesel from entering. Mike Pole said <br /> that the storm drain west of the impacted storm drain had approximately 1-1.5 <br /> inches of what appeared to be diesel in it. <br /> Mike Pole stated that the diesel did not go past that point into the street and <br /> stopped at the sand filter. Storm drain leads to <br /> Mistlen Park retention pond. Spoke with Antoinette of Flying J. She said that a <br /> clean-up company was coming out but was unsure if they would be cleaning up <br /> the entire spill. Called Joey Cupp (Flying J CERS Environmental Contact) at <br /> 865-474-2826 and left a message regarding the spill. Received a call from Chip <br /> Hughes (865-206-5269) of Flying J and he said that a clean-up company is en <br /> route to clean the surface area and the impacted storm drain. Estimated time of <br /> arrival is unknown at this time. Tim Babcock with Broadbent would call me to let <br /> me know what was going on. <br /> Generated complaint. Wrote Notice to Abate and sent it to Chip Hughes (Project <br /> Manager) at Chip.Hughes@pilottravelcenters.com. Completed Emergency <br /> Response Record and Prop 65. <br /> Verified GIS in EC. Received a call from Tim Babcock of Broadbent. He said that <br /> the clean-up ETA is 2:00 pm. Pondor and NRC will be coming out to do the <br /> clean-up. Gave him information on the spill and Mike Pole's contact information. <br /> Received an updated call from Mike Pole that about a cup of diesel was observed <br /> in the first manhole in the street. The second manhole along the line was dry. <br /> Returned to office. Completed site map. Turned in Prop 65 to Haza Saeed (Lead <br /> Sr. REHS) for review. <br /> Called Ponder Environmental Services at 877-256-8265 to find out contact <br /> information for someone on site. They said they would call me back. Fred <br /> Mitchell called back and said that Ponder is only providing the pump truck and <br /> NRC is doing the actual clean-up. <br /> Spoke with Jose Salazar of NRC (510-882-0069) about clean-up. He said it will <br /> be atleast three hours until clean-up is completed. Passed information onto <br /> Claudia Muro (EHS). <br /> 5105.rpt Page 2 of 5 <br />
