Complaint Investigation Summary Report#:5105
COMPLAINT ID: CONKGITOF Site Location: 1501 N JACK TONE RD
Activity Summary
Activity Date Recorded by
09/24/2020 Received complaint information from Lydia Baker(EHD) for cleanup verification. MURO
Discussed with Lead Senior Elena Manzo (EHD) and Lydia Baker(EHD).
Reviewed photo log provided by Lydia Baker.
09/24/2020 6:50 PM Called Jose Salazar (NRC) at 510-882-0069 and left message asking MURO
him to call me back with the cleanup status and a reminder that I will need to be
there to verify the site has been cleaned.
7:10 PM Jose Salazar called, the cell phone only rang once.
7:11 PM I called Jose Salazr back, but call went to voice mail again. I left another
message stressing the importance that he calls back with the status.
7:42 PM Made contact with Jose Salazar, who reported cleanup was complete
and they were leaving the site. I told him if he can wait, since I have to verify the
cleanup. He told me his supervisor/manager Jim told the crew they had to leave
because they ran out of hours. He provided Jim's(NRC) phone number so that I
can call him.
7:49 PM I called Jim at 530-513-0468 left message to call me back.
7:51 PM Jose Salazar called me to report they were going to wait for me.
7:52 PM Headed out for spill site.
8:19 PM Arrived at spill location. Met with Jose Salazar(NRC).
Jose Salazar escorted me to the storm drain that received the greatest impact
which was across the fueling dispensers. I observed there to be no sheen on the
asphalt, storm drain lid, walls inside storm drain, bottom of storm drain, or line. no
smell of diesel was detected. Jose held flashlight inside storm drain as I took
photo. I noted there was a blue hue in the photo, but not present in the storm
drain. I later tested flash with a mirror and found the flash to have a blue hue to it. I
further walked to the storm drain next in line to the west. No sheen or diesel smell
was detected. I was unable to go to the street storm drain as the seemed to be
many freight liner trucks and with no traffic control, I did not feel safe as it was
night time. I asked Jose if they can send me a photo of that storm drain, he said
the person picking up the waste tomorrow will.
Jose told me they power washed the lines, all water was collected and generated
1000 gallons of diesel-contaminated water with was onsite in a frac tank.
The asphalt was cleaned using absorbent, absorbent socks, and absorbent pads.
Once solids were removed, area was then triple rinsed with simple green. All water
collected. Three 55-gallon drums of diesel-contaminated solids was generated. Al
waste is pending analytical testing.
NRC to pick up the three 55-gallon drums tomorrow, 9-25-20 on hazardous waste
manifest. Ponder Environmental to pick up frac tank with diesel-contaminated
water.
Cleanup was deemed adequate.
Generated photo log.
9:50 PM End.
5105.rpt Page 3 of 5
