Site Health and Safety Plan <br />Countryside Market, Stockton, California March 21, 2014 <br />2.0 PROJECT PERSONNEL <br />Apex-SGI and subcontractor personnel will act in accordance with applicable federal, state, <br />regional, and local regulations during all phases of the project. All subcontractor personnel working <br />on the Site are responsible for following the health and safety procedures specified in this HASP <br />and for performing their work in a safe and responsible manner. At the time of job assignment, <br />special training will be provided to Site personnel who may be exposed to unique or special <br />hazards. The training and medical requirements and responsibilities for Site personnel are <br />discussed below. <br />All subcontractor personnel assigned to this project shall have appropriate training and medical <br />clearance. The subcontractor supervisor or manager is required to sign the Subcontractor Training <br />and Medical Clearance Record Form (Appendix A). Subcontractor personnel without proper <br />training and medical clearance will not be allowed to work on the Site. <br />2.1 Apex-SGI Key Personnel <br />Each person on the Site has responsibility for their own health and safety, as well as assisting <br />others in carrying out the HASP. Any person observed to be in violation of the HASP should be <br />assisted in complying with the HASP, or reported to the Site Health and Safety Coordinator. Any <br />Site personnel may shut down field activities if there is a real or perceived immediate danger to life <br />or health or the environment. <br />The implementation of health and safety protective measures at the Site will be an integrated effort <br />among the Apex-SGI Project Manager, the appointed Site Health and Safety Coordinator, and the <br />Health and Safety Director. The specific personnel that will fill these roles for this project are <br />identified below. <br />Project Manager: Greg McIver <br />Site Health and Safety Coordinator: Adam Brown <br />Health and Safety Director: Joe Schmids <br />2.1.1 Project Manager <br />The Project Manager is, by definition, the individual who has primary responsibility for ensuring <br />health and safety compliance on this project. The Project Manager, therefore, is ultimately <br />responsible for implementing the requirements of this HASP.
