Site Health and Safety Plan <br />Countryside Market, Stockton, California March 21, 2014 <br />Ensuring that subcontractor personnel assigned to this project have appropriate training <br />and medical clearance. The subcontractor supervisor or manager is required to sign the <br />Subcontractor Training and Medical Clearance Record Form (Appendix A). <br />Verifying all utility clearances. <br />Maintaining regular communication with the Site Health and Safety Coordinator. <br /> <br />2.1.2 Site Health and Safety Coordinator <br />The Site Health and Safety Coordinator is responsible for enforcing the requirements of this HASP <br />once Site work begins and has the authority to immediately correct all situations where <br />noncompliance with this HASP is noted, including the immediate stoppage of work in cases where <br />an immediate danger is perceived. Some of the Site Health and Safety Coordinator's specific <br />responsibilities include: <br />Ensuring that all Site personnel (Apex-SGI, subcontractor, visitor) have read and <br />understand this HASP and have completed the HASP Acknowledgement and Agreement <br />Form (Appendix B). <br />Conducting a tailgate safety meeting (Appendix C) for all Site personnel (Apex-SGI, <br />subcontractor, visitor), prior to performing work at the Site, apprising them of the contents of <br />the HASP and potential site-specific hazards. <br />Assuring that sufficient personal protective equipment (PPE), as required by this HASP, is <br />available at the Site. <br />Calibrating air monitoring instrumentation, performing air monitoring, and maintaining air <br />monitoring logs, if required. <br />Setting up and maintaining the personnel decontamination facility. <br />Notifying the Project Manager of all noncompliance situations. <br />Supervising and monitoring the safety performance of all personnel to ensure that required <br />health and safety procedures are followed, and correcting any deficiencies. <br />Conducting near miss and incident investigations and preparing appropriate reports <br />(Appendix D). <br />Initiating and supervising emergency response procedures. <br /> <br />2.1.3 Health and Safety Director <br />The Health and Safety Director is the individual responsible for the interpretation and modification <br />of the HASP. Modifications to this HASP which may result in less stringent precautions cannot be <br />undertaken by the Project Manager or the Site Health and Safety Coordinator without the approval <br />HASP 21MAR2014 <br /> 2-2 <br /> APEX
