Site Health and Safety Plan <br />Countryside Market, Stockton, California March 21, 2014 <br />9.0 EMERGENCY RESPONSE PLAN <br />The following sections describe the emergency response plan for safe and effective responses in <br />the event of an emergency while performing Site activities. <br />9.1 Near Miss or Incident <br />If a near miss or incident occurs, take the following action: <br />Notify the Site Health and Safety Coordinator immediately. The Site Health and Safety <br />Coordinator is responsible for immediately notifying the Project Manager, and preparing <br />and submitting a Near Miss Report or Incident Report to the Health and Safety Director <br />within 24 hours. Near Miss Report or Incident Report forms are available in Appendix D for <br />use in the field at the time of the near miss or incident, but it must also be completed online <br />within 24 hours at httplintranet.thesourcegroup.net. <br />9.2 Injury or Illness <br />If an injury or illness occurs, take the following action: <br />Summon for help. If emergency personnel (fire/ambulance) are necessary, call 911. If it <br />makes sense to take an individual to the hospital, see Figure 3 for route to nearest hospital. <br />Begin first aid for the person immediately. Use and complete the First Aid Assessment <br />Form in Appendix D. <br />Notify the Site Health and Safety Coordinator. The Site Health and Safety Coordinator is <br />responsible for immediately notifying the Project Manager, and preparing and submitting an <br />Incident Report to the Health and Safety Director within 24 hours, as well as notifying the <br />employee's supervisor and Principal-in-Charge. Incident Report forms are available in <br />Appendix D for use in the field at the time of the incident, but it must also be completed <br />online within 24 hours at http:/intranet.thesourcegroup.net. <br />If a subcontractor employee is injured, the Subcontractor Supervisor or Manager will also <br />complete their own injury/illness investigation and submit a copy of their report to Apex- <br />SGI's Health and Safety Director as well. <br />The Site Health and Safety Coordinator will assume charge during a medical emergency <br />until more qualified emergency response personnel arrive at the Site. <br />In the event of a medical situation, NOT requiring an ambulance (i.e., minor lacerations, minor <br />sprains, etc.): <br />A <br />HASP 21MAR2014 <br /> 9-1 <br /> APEX
