Box 1692 <br />Manteca, CA 95336 <br />NOTICE OF APPLICABILITY- ORDER R5-2015-0012-0021, BOULEVARD AUTOMOTIVE <br />SERVICE, 2151 COUNTRY CLUB BOULEVARD, STOCKTON, SAN JOAQUIN COUNTY, <br />LUSTIS No. 390044 <br />On behalf of Mr. Roger Hartley, Advanced Geo Environmental, Inc. (AGE) submitted a Notice of <br />Intent (N01) requesting coverage under Waste Discharge Requirements General Order <br />R5-2008-0149 General Waste Discharge Requirements for In-Situ Groundwater Remediation at <br />Sites with Volatile Organic Compounds, Nitrogen Compounds, Perchlorate, Pesticides, Semi- <br />Volatile Compounds and/or Petroleum Compounds. Based on information in your submittals, it <br />is our determination that this project meets the required conditions to be approved under the <br />more current Order R5-2015-0012 Waste Discharge Requirements General Order For In-Situ <br />Groundwater Remediation and Discharge of Treated Groundwater to Land (General Order). As <br />such you are assigned Order No. R5-2015-0012-0021. Boulevard Automotive Service (BAS) is <br />currently monitoring groundwater quality in response to the site's petroleum hydrocarbon plume <br />in groundwater. This approved remediation project as described herein will require additional <br />groundwater monitoring in accordance with enclosed Monitoring and Reporting Program (MRP) <br />No. R5-2015-0012-0021. <br />Project Location: <br />The site is currently used as an automotive repair facility at 2151 Country Club Boulevard in <br />Stockton, San Joaquin County (Figure 1). The legal description for the site is assessor parcel <br />number 123-080-30, Latitude 37.9659 N, Longitude 121.3298W. The closest surface water <br />body is the Smith Canal located approximately 1,300 feet south of the site. The project will be <br />implemented on the Site property and include monitoring from wells located on Elmwood <br />Avenue and Franklyn Avenue up to approximately 150 feet from the Site. <br />Project Description: <br />In April 1985, BAS removed two 1,000-gallon gasoline underground storage tanks (USTs) from <br />this site. Soil and groundwater samples collected from beneath the USTs indicated the <br />presence of petroleum hydrocarbons, primarily total petroleum hydrocarbons as gasoline <br />(TPHg), benzene, toluene, ethylbenzene, and total xylenes. From 1996 to 2011, BAS <br />completed a multiple phases of soil and groundwater investigation; however, petroleum impacts <br />remain in groundwater beneath the Site.
