Former Boulevard Automotive - 2 - 20 October 2016 <br />2151 Country Club Blvd. <br />BAS and its consultant, AGE, propose to inject a solution of hydrogen peroxide into the <br />groundwater at 18 points (DPE-1 and DPE-2, nine proposed shallow depth injection wells, and 7 <br />proposed intermediate depth injection wells) around and downgradient of the former UST <br />locations (Figures 1 and 2) in accordance with the 11 March 2014 Corrective Action Plan <br />prepared on your behalf by AGE. As indicated in the 13 July 2012 In-Situ Chemical Oxidation <br />Pilot Study Report, in-situ chemical oxidation is a viable remedial approach for the Site. <br />Analytical testing will be performed on select monitoring wells in accordance with the enclosed <br />MRP to assess the effectiveness of the remediation effort and to ensure breakdown products <br />generated or mobilized from the injections do not exceed water quality objectives and <br />groundwater limitations based on naturally occurring background water quality. <br />As indicated in the attached MRP, if groundwater quality is degraded and exceeds values in the <br />General Order due to the in-situ chemical oxidation remediation, a contingency plan will be <br />implemented. The contingency plan states that injections will be terminated and wells exceeding <br />the General Order limits will be bulk purged until baseline conditions are attained. <br />No comments were received on the Corrective Action Plan during the 30-day public comment <br />period that ended on 8 August 2014. <br />General Information: <br />The project will be operated in accordance with the requirements contained in the <br />General Order No. R5-2015-0012 and in accordance with the information submitted in <br />the Notice of Intent, and otherwise as specified in this Notice of Applicability (NOA). <br />The required annual fee (as specified in the annual billing you will receive from the State <br />Water Resources Control Board) shall be submitted until this NOA is officially rescinded. <br />Injection of material other than those specified in the NOI into the subsurface is <br />prohibited, unless analysis, as specified in General Order No. R5-2015-0012, of the <br />injectant is provided and approval is granted by Central Valley Water Board staff. <br />Failure to abide by the conditions of the General Order and this NOA can result in <br />enforcement actions as authorized by provisions of the California Water Code. <br />The Discharger shall comply with the attached MRP and any revisions thereto as <br />ordered by the Executive Officer or directed by Central Valley Water Board staff. <br />If you have any questions regarding this NOA, please contact Christopher Flower at <br />(916) 464-4836, or by e-mail at chris.flower@waterboards.ca.gov. <br />LL- 61! <br />( <br />PAMELA CREEDON <br />Executive Officer <br />kL) <br />Enclosure: General Order R5-2015-0012 and MRP <br />cc: Ms. Jovel Vossler, Regional Water Quality Control Board, Rancho Cordova <br />Ms. Angelica Mann, San Joaquin County Environmental Health Department, Stockton <br />Mr. Daniel Villanueva, Advanced GeoEnvironmental, Inc., Stockton
