CHOW ENGINEERING, INC. <br />Rev. 6, 8-8-14 <br /> <br />4.2 Physical Hazards <br />The nature of the work will include utilization of mechanical equipment as well as <br />physical labor at the project site. Hazards will include common slips, trips and falls, as <br />well as awareness requirements common to the operation of mechanical equipment. <br />The work will potentially involve the use of the following equipment: <br />Hollow-stem auger drill rig <br />Equipment including Watera pump and portable generator. <br />Each of the operations utilizing the above described equipment represents the potential <br />for physical injury. Employees and subcontractors will remain alert when performing <br />any physical activity on site and will employ the "Buddy System" at all times. Only those <br />personnel who have been trained and are familiar with the operation of the equipment <br />to be used on site will be allowed to operate this equipment. Supervisory personnel <br />will ensure all safety measures are implemented. <br />All general site operations will be further guided by the provisions of CE SOP #H&S <br />002— "Safe Work Practices", hereby incorporated by reference into this document. As a <br />requirement on all projects, CE personnel will bring to the work site, an industrial first <br />aid kit and sufficient fire extinguishers, commensurate with the work being performed, <br />to effectively provide an adequate first response. <br />On site utilities shall be marked in accordance with current and applicable regulatory <br />requirements. Equipment operators, site supervisor, and subcontractors must <br />physically view the utility locations BEFORE any stages of demolition/excavation work <br />begins. If at any time during the work, CE personnel have any safety concerns about <br />the work, they have the authority to stop work until safety concerns are alleviated and <br />all safety measures fully implemented. <br />There are no confined spaces in at the SWMU #21 site. <br /> <br />4.3 Other Hazards <br />HEAT & COLD STRESS: The physical effects of heat & cold stress can be very <br />hazardous & physically debilitating. Review carefully CE's Procedure on Heat Stress and <br />on Cold Stress as applicable to the weather conditions and type of work being performed. <br />BIOLOGICAL HAZARDS: There is the potential exposure to biological hazards from <br />wildlife at the site. SWMU #21 is in an area with parking and pallet storage so there is <br />low potential for snakes and spiders to be in shaded areas. The monitoring wells are <br />located in parking and driveway access areas which minimizes exposure to biological <br />hazards. <br />4
