CHOW ENGINEERING, INC. <br />Rev. 6, 8-8-14 <br /> <br />4.4 Exposure Monitoring Plan <br />The site hazards and hydrocarbon-related mineral oil fractions in the groundwater are <br />low-level with minimal impact of any kind on site personnel, based on site LEL/02 tests <br />and laboratory analysis. If deemed necessary, in the event that a respiratory hazard <br />becomes apparent, respiratory protection would be implemented for workers and air <br />monitoring procedures implemented. Site personnel would be required to adhere to <br />decontamination procedures as described above. <br /> <br />5.0 Personnel <br /> <br />5.1 Personal Protective Equipment (PPE) <br />Based on the information provided, the established PPE level during field activities will <br />be performed in Level "D". Hard Hats, safety shoes, and safety glasses or face shields <br />at a minimum will be worn by sampling technicians and supervisors. Level "D" gear <br />would also be worn by all other site personnel, the drilling crew and within the zone at <br />the drill rig. CE will establish and maintain a control zone surrounding the work area. <br />All personnel will observe proper decontamination procedures when entering or leaving <br />the zone. Non-disposable uniform items such as work boots, tools and equipment shall <br />be thoroughly decontaminated prior to leaving the work site. <br />CE will utilize field investigation and monitoring techniques that may include <br />instrumentation as well as visual observations to evaluate site conditions with regard to <br />further possible upgrades of PPE protection levels. <br /> <br />5.2 Training Requirements <br />All CE personnel who will be tasked with site activities including groundwater sampling <br />will be 40-hour OSHA trained per 29 CFR 1910.120 with added 8-hour supervisory and <br />onsite training for supervisors. <br />Employees of subcontractors will be trained similarly to perform any site duties as <br />required. All personnel who access the site to perform related activities will also receive <br />training and site instructions during the Tailgate Safety Meetings at the beginning of the <br />work session. <br /> <br />5.3 Medical Surveillance Program <br />The low level risks involved in site work applications do not meet the threshold for a site <br />specific medical surveillance program. If site conditions change and the groundwater <br />concentrations increase, such a program would be considered for implementation. <br />5
