T he route to the nearest hospital from the job site is <br />provided in Figure 3, and turn-by-turn directions are provided in Figure 4. <br />St. Joseph's Medical Center <br />1800 North California Street <br />Stockton, CA 95204 <br />(209) 943-2000 <br />6.0 ORGANIZATIONAL STRUCTURE AND RESPONSIBILITIES <br />In compliance with 29 CFR 1910.120 (b)(2), 8CCR 5192 and OSHA, the following individuals are assigned <br />specific responsibilities and lines of c ommunication for the duration of this project. ALL Em ployees and <br />workers on this project are expected to maintain vigilance at all times to ensure that the work is conducted in <br />a safe and efficient manner. <br />6.1 HEALTH AND SAFETY OFFICER <br />Benjamin Curry is the EF I Global Health and Safety Officer an d has the responsibilit y and authority to <br />oversee the development of this site Health and S afety Plan and to audit the equip ment and training of <br />involved coin pany and s ub-contractor em ployees t o im plement the Work Plan. He or his designat ed <br />representative has discretionary authority to immediately suspend work until further notice. <br />6.2 PROJECT MANAGER <br />Brian Martasin is the desi gnated Project Manager fo r all operations on this project. He is responsible for <br />overall ad ministration of the project activities. His duties include project planning, comm unications, and <br />coordination. He may also assist the P roject Supervisor in the preparation of the Health an d Safety Plan. <br />The Project Manager reports to EFI Global's Health and Safety Officer. <br />6.3 PROJECT SUPERVISOR <br />Matthew Sonnenberg is the designated Project Supervisor for this project and is responsible for verification <br />and overall compliance with this Health and Safety Plan. Duties include, but are not limited to management <br />of staff assignments for <br />On site determination of appropriate levels of personal protective equipment (PPE); <br />Site surveillance, hazard identification, and health risk analysis; <br />Implementation of procedures and programs to eliminate risk to site personnel; <br />Implementation of site control measures; <br />Conducting daily Health and Safety meetings; and <br />Instructing all site personnel in the terms and conditions of this Health and Safety Plan. <br />6.4 SITE HEALTH AND SAFETY OFFICER <br />The Site Health and Safety Officer reports di rectly to t he Project Supervisor or his design ated <br />representative.
