Complaint Investigation Summary Report#:5105 <br /> COMPLAINT ID: COHSTLEBZ Site Location: 2101 E EARHART AVE <br /> Activity Summary <br /> Activity Date Recorded by <br /> 05/05/2021 Completed 5/4/2021 narrative. ALANIZ <br /> Called Dave Myers and discussed the notice to abate. He said that Bagley <br /> Enterprises was going to be cleaning up the waste. <br /> Received a call from Andy Bagley. Andy was going to go to the site today to <br /> investigate the system. He said he would call me when he arrived on site. The <br /> current cleanup plan is to remove the debris from the oil-water separator drain and <br /> assess if any diesel fuel went into the surrounding piping. <br /> Discussed spill with Melissa Nissim, Unit III program coordinator, and Stacy <br /> Rivera, lead senior. <br /> Received an SDS sheet from Dave. <br /> 10:30 AM SITE VISIT <br /> Left for the site with Melissa and met with Andy. Andy dug 9 gallons of soil out of <br /> the oil-water separator drain. <br /> Did not witness any signs that the diesel escaped the oil-water separator drain. <br /> The system had a catch basin to prevent the spread of the diesel. <br /> Left the site at 11:42 AM. <br /> Completed prop 65 <br /> 05/13/2021 Called David Myers and discussed waste disposal. He said is going to have ALANIZ <br /> Safety-Kleen pick up the waste (two 5 gallon containers filled with diesel debris) at <br /> the Ag Center. <br /> Called Stockton Petroleum and spoke with Jason Steele. The absorbent has been <br /> disposed of and he is going to send me the disposal record tomorrow. Sent an <br /> email to Jason with my contact information. <br /> Jason's email: <jsteel@spcfuels.com> phone: 209-915-8221 <br /> 05/19/2021 Received a disposal record from Jason Steel on 5/14/2021 and I received a ALANIZ <br /> disposal record from David Myers on 5/19/2021. Both parties disposed of the <br /> diesel contaminated debris at the San Joaquin County Household Hazardous <br /> Waste facility. <br /> Completed photo logs from 5/4 and 5/5 site visits. <br /> Gathered documents together and abated the complaint. Gave to lead senior for <br /> review. Emailed David letting him know that the complaint was abated. <br /> 05/24/2021 Corrected 5/19/2021 daily and posted coordinates in EC. ALANIZ <br /> 5105.rpt Page 2 of 2 <br />
