All employees shall be trained in safety procedures in the event of a <br />release or threatened release of a hazardous material, including, but not limited to, familiarity with the following <br />plans and procedures: <br />(A) Immediate notification contacts to the appropriate local emergency response personnel and to the unified <br />program agency. <br />(B) Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br />persons, property, or the environment. <br />(C) Evacuation plans and procedures, including immediate notice, for the business site. <br />This training shall be documented electronically or by hard copy and shall be made available for a minimum of three <br />years. <br />CORRECTIVE ACTION: Immediately provide employee training for appropriate personnel and submit a copy of the <br />training records to the EHD. <br />This is a repeat violation, Class II. <br />Overall Inspection Comments: <br />Conducted in -office Hazardous Materials Business Plans (HMBP) inspection. The food inspector, Victor <br />Acevedo, performed an on-site inspection for the CO2 tank on January 19, 2021. There were 6 violations <br />during the in -office inspection. Complete and submit a copy of the Return to Compliance Certification form to <br />the EHD with a statement documenting the corrective actions that have been or will be taken for each violation, <br />and any supporting paperwork, by July 16, 2021. <br />Requests for submittal of employee training records was completed on 6/9/21 on 6/16/21. Employee training <br />Records have not been received. <br />Starting September 1, 2018, all in -office CERS help will be provided at EHD hourly rate ($152).
