Complaint Investigation Summary Report #: 5105 <br />COMPLAINT ID: C00051676 Site Location: 5100 HVVY 12 <br />Activity Summary <br />Activity Date <br />08/31/2020 Received status request from Lead Senior Elena Manzo (EHD) for facility that had <br />complaint involving chemicals for dust control. <br />Typed up email asking whether the chemicals the facility's use of the following <br />chemicals: Magnesium Lignosulfonate Solution, Intermediate, & Heavy; and Plaza <br />48% Solids Red Liquor (the that is material premixed into the Magenesium <br />Lignosulfonate) were still being used. I also asked if they have further researched <br />the chemicals. <br />09/02/2020 The email I wrote to Henry Rotor (Alegre Trucking) on 08-31-20 was still in my <br />Drafts. I sent the email and received a reply from Henry Rotor. Henry reported the <br />facility has not used the chemicals in question. He asked that I give him a call at: <br />209-810-5209. <br />I called Henry Rotor. He wanted to know if everything was okay with the complaint. <br />I told him we are in the process of abating the complaint and it was just something <br />that needed to be included in the complaint file. I let him know that we will be <br />cross-referencing the program and complaint. The disposal records for the waste <br />generated will be reviewed on their next routine inspection. He then told me he <br />already emailed the disposal records to us. I told him I didn't receive a copy. He <br />said he emailed to Elena Manzo (Lead Senior REHS at EHD). I asked if he could <br />forward that email to me. He said he will do that tomorrow since he is currently <br />unavailable. He asked that I email him the request for the disposal records so that <br />he does not forget. <br />I emailed Henry Rotor a request (Henry@alegretrucking.com) for a copy of the <br />disposal record. <br />I also completed corrections/edits as requested by Lead Senior Elena Manzo <br />(EHD) (handwritten on log2 report) for clarification and details. <br />Recorded by <br />MURO <br />MURO <br />09/08/2020 Reviewed email from Henry Rotor received on 08-03-20. MURO <br />Henry stated that the manifest from the original date sent out to disposal. It was <br />shipped with the facility's normal pickup. <br />Henry attached a scan of a manifest that had barely legible manifest number, date <br />of transport, and customer and transporter signatures. <br />Searched HVVTS for manifest 030010609WAS, and variations similar to. <br />Searched manifest report on RCRAonline for all manifests from this facility; did not <br />find the final copy. <br />09/15/2020 Emailed Henry Rotor a request for final manifest. MURO <br />Generated photo log of cleanup verification. Printed ER01, narrative, disposal <br />manifest, 2 photo logs, updated complaint investigation form, and emails. <br />5105. rpt Page 5 of 6
