• <br />STATE OF CALIFORNIA <br />DEPARTMENT OF CALIFORNIA HIGHWAY PATROL <br />HAZARDOUS MATERIALS INCIDENT REPORT <br />CHP 407E <br />Page 4 <br />SEQUENCE OF EVENTS: <br />On May 13, 2018, at approximately 0338 hours, a Peterbilt truck tractor with semi-trailer was <br />traveling on southbound 1-5 north of the Kasson Road overcrossing, in the #2 lane, at <br />approximately 55 mph. A Dodge pickup was traveling northbound in the southbound #1 lane at <br />an undetermined rate of speed. The Dodge swerved into the #2 lane and collided with the front <br />of the Peterbilt. The Peterbilt came to rest down the west dirt embankment and the Dodge <br />came to rest down the west dirt embankment north of the Peterbilt. As a result of the collision, <br />the Peterbilt fuel tank was ruptured and the Peterbilt caught on fire. Approximately 20 gallons <br />of fuel leaked onto the dirt embankment. The driver of the Dodge suffered minor injuries to his <br />hands. Southbound 1-5 was closed at 11th Street. Traffic was diverted to 11th Street then to <br />Kasson Road and onto southbound 1-5. Cal-Trans were notified at approximately 0416 hours <br />and San Joaquin County Environmental Health and OES was notified at approximately 0902 <br />hours. Tracy Commander Lt. David Wharry, ID 14792, was notified at approximately 0429 hours <br />and arrived on scene at approximately 0815 hours and assumed incident command. Lt. Wharry <br />relinquished incident command authority to Sergeant R. Rickman, ID 14921, at approximately <br />1130 hours. Sergeant Rickman remained on scene until the scene was declared safe. The <br />registered owner of the Peterbilt contacted Environmental Logistics to arrange clean up at a <br />later date. San Joaquin Environmental Health Department Environmental Health Specialist <br />Garrett Backus and Cal-Trans District Hazardous Material Manger Juan Armendarez, spoke to <br />Marcial Barragan of Environmental Logistics. Environmental Logistics will complete a USA <br />survey and once complete, will notify Garrett Backus and Juan Armendarez prior to hazardous <br />material site clean-up. Garrett Backus of San Joaquin County Environmental Health declared <br />the scene safe at approximately 1420 hours. Southbound 1-5 reopened at approximately 1432 <br />hours. <br />ROAD CLOSURES: <br />Southbound 1-5 was closed at 11th Street. Traffic was diverted to 11th Street then to Kasson <br />Road and onto southbound 1-5. Southbound 1-5 reopened at approximately 1432 hours <br />EVACUATION DETAILS: <br />None required.
