STATE OF CALIFORNIA <br />DEPARTMENT OF CALIFORNIA HIGHWAY PATROL <br />HAZARDOUS MATERIALS INCIDENT REPORT <br />CHP 407E <br />Page 5 <br />ENVIRONMENTAL IMPACT: <br />Approximately 20 gallons of diesel fuel spilled onto the soil located west of the freeway. The <br />containment was approximately 10 feet in circumference. Office of Emergency Services was <br />notified at approximately 0902 hours and issued OES #18-3089. San Joaquin County <br />Environmental Health was notified at approximately 0902 hours and arrived on scene at <br />approximately 1338 hours. <br />CLEAN UP ACTIONS: <br />San Joaquin Environmental Health Department Environmental Health Specialist Garrett Backus, <br />and Cal-Trans District Hazardous Material Manger Juan Armendarez, spoke to Marcia! Barragan <br />of Environmental Logistics. Environmental Logistics will complete a USA survey and once <br />complete will notify Garrett Backus and Juan Armendarez prior to hazardous material site <br />clean-up. Garrett Backus of San Joaquin County Environmental Health declared the scene safe <br />at approximately 1420 hours. <br />ACTIONS OF OTHER AGENCIES: <br />American Medical Response (AMR) responded to the scene at approximately 0354 hours. <br />AMR transported the driver of the Dodge to San Joaquin County Hospital. <br />Tracy Fire Department, Engine 98 and Water Tender 92, arrived on scene at approximately <br />0352 hours. Engine 93 arrived on scene at approximately 0353 hours and Battalion 9 and Truck <br />91 arrived on scene at approximately 0356 hours. Tracy Fire Department extinguished the fire <br />to the Peterbilt and provided medical assistance. <br />Cal-trans respond to the scene at approximately 0430 hours, conducted the road closure and <br />freeway clean up actions. <br />Area Superintendent Esther Herschbach <br />Maintenance Area Superintendent John Miller <br />District Hazardous Material Manager Juan Armendarez <br />San Joaquin County Environmental Health Department Environmental Health Specialist <br />Garrett Backus responded to the scene at approximately 1338 hours and supervised the clean- <br />up. Specialist Backus declared the scene safe at approximately 1420 hours.
