The designated operator failed to check and note all required items on the visual inspection reports. The incorrect and missing information included the following from the inspection reports: -Section V was filled out by the designated operator and did not include the corrective actions that will be taken by the owner/operator. -The owner/operator printed name was left blank. -The designated operator phone number was left blank. -The NA answers were not included in section III. -Tank and line tightness testing information was left blank. During the visual inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and shall check all required items on the report. Ensure that designated operators performing visual inspections at this facility are including all of the required information on the reports. The designated operator failed to document all the alarms from the attached alarm history on the designated operator inspection reports and failed to check that they were responded to appropriately. The missing alarms include: -L1 (diesel STP sump)fuel alarms on 2/26/19 and 4/16/18 -L5 (UDC 13/14)fuel alarms on 2/27/19 and 11/23/19 During the inspection, the designated operator shall review the alarm history since the previous inspection, check that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the report. Ensure that designated operators performing inspections at this facility are including all of the required information on the reports. This is a Class II violation. 210 CCR 2716(f) DO monthly reports not maintained on site for 12 months or for 36 months (after October 1, 2018). Designated operator inspection reports were found for 2/6/19 and 4/3/19, but none was found for the inspection in between those dates. Designated operator inspection reports for the previous thirty-six months shall be retained on site. Locate and ensure that copies of the previous thirty-six months of designated operator inspection reports are maintained on site. Submit copies of the missing reports to the EHD.
