The designated operator shall train facility <br /> employees for which he or she is responsible in the proper operation and maintenance of the UST system once every <br /> 12 months. The training shall include, but is not limited to: <br /> 1. Operation of the UST system in a manner consistent with the facility's best management practices <br /> 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br /> 3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br /> 4. Name of the contact person(s)for emergencies and monitoring equipment alarms <br /> Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of <br /> the training records to the EHD. <br /> This is a Class II violation. <br /> 315 HSC 25291(e)Water in secondary containment not removed, analyzed, and properly disposed of (pre-Jul 2003). <br /> Liquid was observed in UDC 16. If water could enter into the secondary containment by precipitation or infiltration, it <br /> must be removed and disposed of properly. Immediately remove this liquid, make a hazardous waste determination <br /> per Title 22 hazardous waste regulations, and manage it accordingly. Ensure that all UDCs are maintained free of <br /> liquid. <br /> This is a repeat violation, Class II. <br /> 320 CCR 2635, 2637.2, 2665, 2712 Failure to comply with the installation, inspection, or record keeping of the overfill <br /> prevention. <br /> On the 8/29/18 Overfill Prevention Inspection Results for the mechanical flappers, the tank diameters are documented <br /> as smaller than the level at which the flappers are set to activate. Notification was provided to EHD for overfill <br /> prevention testing to be conducted on 4/11/19; no test results were found for this testing. <br /> All UST systems must be equipped with an approved, functional overfill prevention device at all times during fuel <br /> deliveries. The failed components must be repaired or replaced by a properly licensed, trained, and certified contractor <br /> under permit and inspection of the EHD. If the failed component can't be replaced immediately, there is a possibility <br /> that the 87, 89, 91 and diesel UST systems may be red tagged to prevent fuel inputs. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> The monitoring system certification was conducted on 4/11/19 and was not witnessed by EHD. Looked inside all <br /> sumps and diesel (truck side) UDCs. Verified sensor locations and piping communication. Verified spill bucket 5 <br /> gallon capacity. The paperwork inspection was conducted on 5/24/19.
