Complaint Investigation Summary Report#:5105 <br /> COMPLAINT ID: CORKVD2LW Site Location: 1000 ROTH RD <br /> Activity Summary <br /> Activity Date Recorded by <br /> 11/23/2020 Discussed with Garret Backus ILead Senior REHS1. Based on discussion I left to GAITAN <br /> go inspect spill area. Contacted Paul Holt, HazMat Supervisor, stated that Mike <br /> Algots, HazMat Supervisor, and himself have received notification regarding the <br /> paint waste. He confirmed that Patriot Environmental was on route to perform <br /> clean up. Paul stated that Mike would be occupied with a derailment in Fresno <br /> County, and that they both would be limited in phone calls. He stated that an <br /> onsite manager should be able to direct me to the spill. <br /> Arrived on site met with Loreen Gastineau, Senior Manager for Terminal <br /> operations, stated that cargo was observed leaking once crane operator lifted the <br /> top half of the double stack cargo container. Crane operator noticed black paint <br /> waste accumulating on the top and sides of the bottom cargo container. Stated <br /> that both containers were immediately transferred to spill containment area. <br /> Loreen stated that UPRR employees inspected the track for any paint waste from <br /> the container location (adjacent to rail car holding area)to Roth Rd. entrance, no <br /> paint waste were observed on the track. I observed the following: <br /> -paint waste on sides of rail car, carrying cargo containers. <br /> -paint waste observed on sides on cargo container; shipping container <br /> (UMXU254598). <br /> -Paint waste observed dripping underneath cargo container; shipping container <br /> (UMXU257619). <br /> I requested if Loreen could notify ER phone once she has an ETA for Patriot <br /> Environmental. Discussed complaint with Elena Manzo ILead Senior REHS1. <br /> Generated Notice to Abate sent to Mike Algots, HazMat Manager. <br /> 11/24/2020 12:10 AM Left home, arrived at Union Pacific Railroad (UPRR). MURO <br /> 12:58 AM Met with Brandon Eck(Patriot Environmental Services Site Supervisor). <br /> Observed Railcar container UMXU254598 to have black paint already dried and <br /> solidified on the exterior lateral surface. Brandon confirmed the paint to be dry by <br /> making contact with his hand along the painted exterior lateral surface. I observed <br /> his hand to be clean without paint. The railcar was no longer onsite. Brandon <br /> showed me a photo of the railcar after they had cleaned it. He told me that most of <br /> the paint had already dried and solidified. I observed the asphalt where Jason had <br /> seen drops of paint. Brandon told me that they used a flat metal scraper to scrape <br /> of the paint which had already dried. The dried paint popped off. They had begun <br /> to reload container UMXU257619, but Brandon had a photo of the container's <br /> interior floor after cleanup. I asked Brandon if anyone had checked the tracks for <br /> any paint leaks. Brandon reported that he went through the yard and checked the <br /> tracks, but did not see any paint drippings. Cleanup for all impacted areas was <br /> adequate. <br /> Left Union Pacific yard at 1:25 AM and returned home at 2:10 AM. <br /> Worked on narrative for 11-23 and began the photo log. <br /> 3:10 AM End <br /> 11/24/2020 Worked on photo log.
