Pathology - Placenta Retention and Eination <br />A. The checklist for placenta examination in delivery will be placed in charts of <br />all inpatient labor patients. <br />B. Nursing staff will stamp the checklist with patient demographic information. <br />C. Nursing staff will complete Section B (Demographic History) of the <br />checklist. <br />D. Physician will complete Section A and sign where indicated. The first copy <br />(original) of the Placenta Examination Form is to remain on the mothers' chart. <br />The second copy of the checklist is to remain on the infants' chart. <br />E. Placentas that meet the criteria for pathological exam should be placed in <br />10% formalin. <br />Note: If the placentas are placed in formalin, the specimens can not <br />be used for microbiologic or other workups. The specimen <br />container is to be labeled with the patient identifier on the side of <br />the container, placed in a transport bag, and delivered to <br />Pathology with the remaining copies of the Placenta Examination <br />Form. <br />F. Placentas not meeting the criteria for pathological exam, will be placed in a <br />container without formalin. The specimen container with the placenta will be <br />labeled with patient identifier both on the lid and side of the container, placed <br />in a transport bag, and delivered with the remaining copies of the Placenta <br />Examination Form to the laboratory for refrigerated storage. <br />G. If the Placenta requires special studies, it should be submitted <br />moistened in saline (not submerged) to Pathology with Placenta <br />Examination Form and Pathology Requisition Form. On Kaiser patients, <br />please indicate if request is for Oakland Kaiser. <br />II. Laboratory Procedure for handling and storage of placentas: <br />A. Receive the placenta and the Placenta Examination Form parts 3 and 4. The <br />placenta must be stored in a properly labeled and sealed, leak -proof container <br />and should not be in formalin or any other fixative. It should not be frozen or <br />left at room temperature for any length of time. <br />B. The "Placenta Examination Form" which is filled out by the delivering <br />physician must be complete and presented with the specimen. The check list for <br />placenta examination will indicate whether the placenta is to be held or is being <br />submitted for pathological examination. All pertinent case information should <br />be included on the placenta examination checklist. This would include Sections <br />A, B, C and must have the physician signature. <br />C. Place the placenta in designated refrigerator. Attach a color coded dot <br />indicating the day of the week the placenta was obtained. This will help facilitate <br />tracking and discard of specimens. <br />Yellow = Monday <br />Green = Tuesday <br />Orange = Wednesday <br />Dark Blue = Thursday <br />Light Blue = Friday <br />Red = Saturday <br />Black = Sunday <br />D. Record delivery of the placenta to Pathology indicate the date and initial of <br />person making the delivery on the "Placenta Examination Form." <br />E. Retain the form in the basket provided near the storage refrigerator.
