Time In: 12:17 pm <br /> Time Out: 1:06 om <br /> COMPLAINT INSPECTION REPORT <br /> Name of Facility: MOUNTAIN MIKES PIZZA Date: 09/02/2021 <br /> Address: 3526 MANTHEY RD, STOCKTON 95206 <br /> Owner/Operator: SINGH, SURINDER PAL&YADWINDER Telephone: (209)983-1853 <br /> Program Element: 1600- FOOD PROGRAM Complaint#: C00054128 <br /> Responsible Party Contact: <br /> DESCRIPTION OF COMPLAINT ALLEGATIONS <br /> The San Joaquin County Environmental Health Department received the following complaint: <br /> C00054128 states: "COMPLAINANT ALLEGES THIS FACILITY IS DIRTY,THERE ARE CIGARETTE BUTTS ON THE <br /> GROUND INSIDE OF THE ESTABLISHMENT, LOTS OF GARBAGE, BEHIND THE COUNTER LOOKS DIRTY,THERE <br /> ARE FLIES ALL OVER THE CUPS USED FOR DINE IN, THEY WITNESSED PEOPLE MAKING ORDERS AND MOVING <br /> ON TO THE NEXT ORDER WITHOUT WASHING THEIR HANDS." <br /> VIOLATIONS AND CORRECTIVE ACTIONS <br /> Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br /> 113700.All violations must be corrected within specified timeframe. Violations that are classified as"MAJOR"pose an immediate threat to public health <br /> and have the potential to cause foodborne illness.All major violations must be corrected immediately.Non-compliance may warrant immediate closure of <br /> the food facility. <br /> OVERALL INSPECTION NOTES AND COMMENTS <br /> FOOD ITEM--LOCATION--TEMP°F--COMMENTS <br /> No Temperature Data Collected <br /> NOTES <br /> Complaint investigation conducted.
