The Environmental Health Department (EHD) completed th <br />amended written report on September 1, 2016, which replaced the checklist left at the facility, and emailed the <br />amended report, along with the checklist completed on August 8, 2016 and photos taken on August 8, 2016, to <br />Hardeep Gill, environmental contact. The September 1, 2016 inspection report has been amended again by the <br />EHD by revising violation 319. <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, within 30 days of receiving the complete inspection report. <br />Note: The EHD "accepted" submittals to California Environmental Reporting System (CERS) on July 21, 2016; thi: <br />means that the submittals were received, much like receiving mail from the postal service. To date, violations fron <br />the June 17, 2016 inspection have not been addressed; they include Items #: 103,104,105, 106, 201, 203, 204, <br />207, portions of 305, 313, 316, and 319. A response that addresses these violations is over a month overdue. <br />Note: S6 sensor fault alarm on June 16, 2016 cleared by June 20, 2016 and has not re -alarmed. <br />Note: Vacuum sensor monitoring of the secondary containment piping of the truck side of the facility of the <br />vacuum/pressure/ hydrostatic (VPH) system was not tested during the inspection today. There is epoxy in the plL <br />such that the plug cannot be opened without damaging the under dispenser containment. <br />Note: Automatic tank gauges (ATGs) are used for overfill protection both on the auto and truck sides of the facility <br />The ATGs were tested today for overfill protection at ninety percent on both the auto and truck sides of the facility. <br />(The facility cannot provide documentation that the flappers are set at ninety-five percent so change "Yes" to "No" <br />for Fill Tube Shut-off Valve under Underground Storage Tank(UST) Tank Information for all five USTs in CERS) <br />Note: The service technician reported today that a new siphon jet valve was installed on June 16, 2016 on the <br />diesel south (truck side) line leak detector. <br />Note: Brine appeared to be clear to faintly pink in color in diesel south submersible turbine pump (STP) sump anc <br />diesel south fill sump on the truck side of the facility; and diesel STP sump, 91 -product fill sump and 87 -product fil <br />sump on the auto side of the facility. <br />Note: Communication in the 87 -product, 91 -product, and three diesel STP sumps and fill sumps and <br />communication in the under dispenser containment (UDC) sumps of the truck side of the facility were completed <br />today. <br />AFTER THE COMPLIANCE DATE, EHD WILL BILL FOR ALL TIME AND ACTIVITIES ASSOCIATED WITH BRINGING THIS <br />FACILITY BACK INTO COMPLIANCE <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE <br />Received by: Date: <br />Inspector: <br />Inspector Phone: <br />a Printed Name and Title: <br />wall l �� <br />VICKI MCCARTNEY, Senior REHS <br />(209) 468-9852 <br />Page 5 of 5 <br />
