Due to time <br />constraints, a checklist was provided to the facility clerk on the second day of the inspection (6/17/16). The <br />Environmental Health Department (EHD) completed the written report on June 21, 2016, which replaced the <br />checklists left at the facility on June 16 and 17, 2016, and sent the inspection report and photos taken during the <br />inspections to the environmental contact, Hardeep Gill. The report has been amended by the EHD by removing <br />violation 313 and revising violation 319. The amended report replaces the initial report. <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, within 30 days of receiving the complete inspection report. <br />Note: Notify the San Joaquin County Environmental Health Department (EHD) what method is being used for <br />overfill protection both on the automobile and truck sides of the facility. <br />Note: Sump collars that had been installed previously were verified for the STP sumps both on automobile and <br />truck sides of the facility during the inspections on June 16 and 17, 2016. <br />Please be aware that as of January 1, 2014, facility operators are required to upload the following UST program <br />documents into the California Environmental Reporting System (CERS): UST Monitoring Site Plan, UST <br />Certification of Financial Responsibility, UST Response Plan, UST Letter from Chief Financial Officer(if <br />applicable), and the Owner Statement of Designated Operator Compliance.
