gallons of diesel were found in UDC sump 16A/17B, and the service technician, Mr. <br /> Fajardin, stated that it was the same diesel found in October because he had never <br /> pumped it out. Mr. Fajardin pumped the diesel out and placed it into one of the diesel <br /> USTs. <br /> During a review of the facility's paperwork, Ms. Henry found the Unified Program <br /> Consolidated Forms (UPCF) for facility information and tank information were not <br /> current, maintenance records and alarm history log were not maintained, and current <br /> financial responsibility documents were not submitted to the EHD. None of the releases <br /> were recorded and reported as outlined on the facility's approved response plan. <br /> On December 24, 2008, Ms. Henry called and left a message for Ms. Christina Gill, Vice <br /> President for B&G Group, Inc. Ms. Henry wanted to meet with Ms. Gill to review the <br /> UST inspection report. <br /> On December 30, 2008, Ms. Henry called Ms. Gill, who stated she just arrived at work <br /> and was dealing with a landscaping problem. Ms. Gill told Ms. Henry she would like to <br /> meet the following week and would call back. Ms. Gill did not call back. <br /> On January 6, 2009, Ms. Henry delivered a copy of the December 22, 2008, UST <br /> inspection report to the facility and sent a copy to the owner via certified mail <br /> (Attachment 11). Attachment 12 includes the alarm histories before and after testing on <br /> December 22, 2008. <br /> On January 9, 2009, a monitoring system certification test report for the October 30, <br /> 2008, testing was submitted two months late (Attachment 13). Test reports are required <br /> to be submitted within 30 days of testing. Also submitted was the December 22, 2008, <br /> monitoring system certification test report (Attachment 14) and a test report for the <br /> components replaced and tested under permit SR0055872 (Attachment 15). <br /> On January 27, 2009, Ms. Henry received a voice mail from Ms. Gill who stated she was <br /> having the monitoring system certification, leak detector testing, and spill container <br /> testing done next week. Ms. Henry called Ms. Gill and asked her why she was having <br /> the testing done. Ms. Gill stated that was what the inspection report indicated. Ms. <br /> Henry was at the annual CUPA conference and did not have the file with her, so she <br /> asked Ms. Gill to read the inspection report to her. Ms. Gill stated she did not have a <br /> copy of the report with her, and Ms. Henry asked her to call back when she had a copy <br /> of the report. Ms. Henry explained that she wanted to meet with her to review the <br /> inspection reports so Ms. Gill would understand what was required. Ms. Henry reminded <br /> Ms. Gill that she still needed to address the violations from all three inspection reports. <br /> On January 28, 2009, Ms. Henry returned a call from Ms. Gill, who referred to the <br /> violations that were noted on all three 2008 inspection reports. Ms. Henry explained that <br /> the testing was currently up to date and that the violations were written because they <br /> were performed late. Ms. Henry asked Ms. Gill if the inspector reviewed all the <br /> requirements with her at the time of the final inspection in 2007. Ms. Gill said yes. Ms. <br /> Henry reminded Ms. Gill that she still needed to address the violations from all three <br /> inspection reports. <br /> On February 18, 2009, permit application SR0056549 was submitted to replace a flex <br /> hose connector in the south diesel piping sump.
