Annual monitoring system certification, leak detector testing, and spill container testing was <br />performed on July 9, 2013 and a test report was submitted to the EHD via email from the Epic <br />Compliance Inc on November 7, 2013, approximately three months past the 30 days of testing. A copy <br />of the test report must be submitted within 30 days of the tests. <br />204. During the monthly inspection, the designated operator shall review the alarm history for the <br />previous month, check that each alarm was documented and responded to appropriately, and attach a <br />copy of the alarm history with documentation taken in response to any alarms to the monthly report. <br />Ensure that designated operators performing monthly inspections at this facility are including all of the <br />required information on the reports. Submit a written statement how to ensure the designated operator is <br />documenting all alarm along with its response action. <br />303. The 91 STP sump sensor was not working properly to ensure continuous monitoring of the tank <br />system as required by CCR 2630(d). The sensor failed to go into alarm when the technician placed the <br />sensor into a cup of liquid. Correct immediately by having a properly licensed, trained, and certified <br />contractor replace the failed component with a functional component (LG 113-x listed, if applicable). <br />-Corrected on site.
