Financial responsibility documents have not been submitted to the EHD for the 2013 year nor for <br />2012, and 2011 year. Current financial responsibility documents are required to be submitted annually. <br />Immediately submit a copy of the financial responsibility documents to the EHD. Alternatively, log into <br />CERS, upload the required information, and submit for review by the EHD. <br />- CERS stated a copies were: Provided to Regulator. EHD received Letter from Chief Financial Officer <br />on August 8, 2013. No Letters from CFO have been received for 2012 and 2011 year. No Certification of <br />Financial Responsibility have been received for 2011, 2012 and 2013 year. <br />109, 110, & 111. During the inspection in June 2012, it was observed that the annual monitoring system <br />certification, leak detector testing, and spill container testing were last performed on June 20, 2012 and <br />was completed on July 9, 2013, one month past due. The two May 2013 Designated Operator (auto and <br />truck side) monthly visual inspection reports noted "AMC & Spill containment test time due next month <br />06/20/13". The two June 2013 Designated Operator (auto and truck side) monthly visual inspection <br />report noted "AMC & Spill Containment tests scheduled - 07/09/13". These tests are required once every <br />12 months. The Designated Operator had provided a written statement a month prior to due date that <br />test is due. Take all necessary precautions to ensure testing is performed in a timely manner. Submit a <br />written statement how to ensure testing is performed on time. <br />- The written corrective action statement received on August 8, 2013 documented incorrectly the test <br />date.
