Annual monitoring system certification, leak detector testing, and spill container testing was performed on July 9, 2013 with Epic Compliance and a test report has not been submitted to the EHD. A copy of the test report must be submitted within 30 days of the tests. Immediately submit a copy of the test report to the EHD. 204. During the inspection in July 2013, it was observed that the designated operator failed to document all the alarms from the attached alarm history on the May 2013 designated operator monthly inspection report and failed to check that they were responded to appropriately. The missing alarms include: L22 Disp 17 Brine Annular Space sensor out alarm that occurred on 04/25/13. Facility employee made a note of the alarm on their own Fast Lane III Alarm Report. During the monthly inspection, the designated operator shall review the alarm history for the previous month, check that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. Submit a work order/maintenance record or a response action for the L22 sensor out alarm. 205. During the inspection in June 2012, it was observed that the designated operator monthly inspection reports for November and October 2011 auto & truck sides were not found on site. Per Designated Operator Brian, who was present on site during the June 2012 testing, their testing company received a letter from operator's bankruptcy lawyer to cease any activities to any of the operator's facilities. Epic Compliance testing company resumed the facility's monthly inspection reports after it was discovered that the letter was incorrectly submitted to them. Locate and ensure that the copies of the previous 12 months of the designated operator monthly inspection reports are maintained on site. Ensure owner/operator review the designated monthly inspection reports on a monthly basis to verify designated operator is completing their inspection. Submit an explanation for the missing November and October 2011 reports. 206. During the inspection in July 2013, it was observed that the Designated Operator failed to note hiring dates on training log. Designated operator lists the names of employees with the date the training occurred along with signature of the Designated Operator but failed to note the hiring dates of each employees. The training list shall include dates of training for all facility employees including the hiring dates of employees hired on/ after July 1, 2005 per CCR, Title 23, 2715(f)(3). Facility and UST was built in 2006. Submit a revised training log with hiring dates for current employees who were trained in 2013 to EHD. - Corrected on site today. A list of current employees name and hire date was provided. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
