If the failed <br />components can't be replaced/ repaired immediately, there is a possibility that the Auto and Truck Side <br />diesel UST system may be red tagged to prevent fuel inputs. Submit an explanation or plans for repairs <br />to the UST system to ensure that the diesel STP sumps and diesel under dispenser containments are <br />free of liquids. <br />203. During the inspection in June 2012, it was observed that all Truck Side diesel dispenser pans, <br />North and South Diesel STP sumps and Auto Side diesel STP sump have what appears to be diesel <br />fuel. Any authorized release from primary containment which UST operator is able to clean up within 8 <br />hours after release was detected or should be detected, and which does not escape from secondary <br />containment, does not increase hazard or fire or explosion, and does not cause any deterioration of <br />secondary containment, must be recorded in facility's monitoring plan. Report information must include: <br />1. Owner's operator's name & telephone number, 21. List of types, quantities, and concentration of <br />hazardous substance released, 3. Description of action taken to control & stop the release, 4. Method & <br />location of disposal of hazardous substance. Attach copies of any hazardous waste manifest, 5. <br />description of proposed methods of repair or replacement of primary or secondary containment, 6. <br />description of additional action taken to prevent future releases. A copy of Recordable Release Report <br />must be submitted to SJC EHD if clean up is within 8 hours after detection. A copy of Reportable <br />Release Report will be submitted if clean up is not done within 8 hours. <br />- Corrected on site. Copies of UST Recordable Release/ Spill Log found on site. <br />Submit to SJC EHD within 30 days (10/11/13): <br />1. one copy of the completed "Return to Compliance Certification" form <br />2. a written corrective actions statement for each of the above violations <br />3. request/ supporting documentation <br />Notes: <br />- Provided copy of free training schedule.
