Immediately discontinue use of failed piping and have a properly licensed, trained and certified contractor replaced or repair the failed piping under permit and inspection of EHD. If the failed components can't be replaced/ repaired immediately, there is a possibility that the Auto and Truck Side diesel UST system may be red tagged to prevent fuel inputs. Submit an explanation or plans for repairs to the UST system to ensure that the diesel STP sumps and diesel under dispenser containments are free of liquids. 203. During the inspection in June 2012, it was observed that all Truck Side diesel dispenser pans, North and South Diesel STP sumps and Auto Side diesel STP sump have what appears to be diesel fuel. Any authorized release from primary containment which UST operator is able to clean up within 8 hours after release was detected or should be detected, and which does not escape from secondary containment, does not increase hazard or fire or explosion, and does not cause any deterioration of secondary containment, must be recorded in facility's monitoring plan. Report information must include: 1. Owner's operator's name & telephone number, 21. List of types, quantities, and concentration of hazardous substance released, 3. Description of action taken to control & stop the release, 4. Method & location of disposal of hazardous substance. Attach copies of any hazardous waste manifest, 5. description of proposed methods of repair or replacement of primary or secondary containment, 6. description of additional action taken to prevent future releases. A copy of Recordable Release Report must be submitted to SJC EHD if clean up is within 8 hours after detection. A copy of Reportable Release Report will be submitted if clean up is not done within 8 hours. - Corrected on site. Copies of UST Recordable Release/ Spill Log found on site. Submit to SJC EHD within 30 days (10/11/13): 1. one copy of the completed "Return to Compliance Certification" form 2. a written corrective actions statement for each of the above violations 3. request/ supporting documentation Notes: - Provided copy of free training schedule. Recommended courses: hazardous & universal waste management, owner/ operators of UST system ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
