Financial responsibility documents have not been submitted to the EHD for the 2013 year nor for 2012 and 2011 year. Current financial responsibility documents are required to be submitted annually. Immediately submit a copy of the financial responsibility documents to the EHD. Alternatively, log into CERS, upload the required information, and submit for review by the EHD. 109, 110, & 111. Annual monitoring system certification, leak detector testing, and spill container testing were last performed on June 20, 2012 and was completed on July 9, 2013, one month past due. The two May 2013 Designated Operator (auto and truck side) monthly visual inspection reports noted "AMC & Spill containment test time due next month 06/20/13". The two June 2013 Designated Operator (auto and truck side) monthly visual inspection report noted "AMC & Spill Containment tests scheduled - 07/09/13". These tests are required once every 12 months. The Designated Operator had provided a written statement a month prior to due date that test is due. Take all necessary precautions to ensure testing is performed in a timely manner. Submit a written statement how to ensure testing is performed on time. 204. The designated operator failed to document all the alarms from the attached alarm history on the May 2013 designated operator monthly inspection report and failed to check that they were responded to appropriately. The missing alarms include: L22 Disp 17 Brine Annular Space sensor out alarm that occurred on 04/25/13. Facility employee made a note of the alarm on their own Fast Lane III Alarm Report. During the monthly inspection, the designated operator shall review the alarm history for the previous month, check that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. Submit a work order/ maintenance record or a response action for the L22 sensor out alarm. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
