Designated Operator failed to note hiring dates on training log. Designated operator lists the names <br />of employees with the date the training occurred along with signature of the Designated Operator but <br />failed to note the hiring dates of each employees. The training list shall include dates of training for all <br />facility employees including the hiring dates of employees hired on/ after July 1, 2005 per CCR, Title 23, <br />2715(f)(3). Facility and UST was built in 2006. Submit a revised training log with hiring dates for current <br />employees who were trained in 2013 to EHD. <br />311. The Auto side Diesel FePetro STP -mechanical line leak detector failed to detect a leak when <br />tested. All line leak detectors shall be capable of detecting a 3 -gallon per hour leak at 10 psi. <br />Immediately have a properly licensed, trained, and certified contractor repair or replace the failed leak <br />detector (LG 113-x listed, if applicable) and obtain a permit within one business day from the EHD. If the <br />failed leak detector can't be replaced immediately, there is a possibility that the Auto Side UST system <br />may be red tagged to prevent fuel inputs. <br />402. Primary containment not liquid tight. <br />- Approximately half an inch of liquid was observed in the Auto Side vent box but insufficient amount to <br />trigger an alarm. Liquid was removed by technicians. <br />- Approximately one cup of diesel was observed in the Auto Side Diesel STP sump. Diesel was removed <br />with a hydrocarbon absorbent pad. <br />- Approximately one and half cup of diesel was observed in each of the Truck Side North and South <br />Diesel STP sump. Diesel was removed with a hydrocarbon absorbent pad. <br />- No measurable diesel liquid was observed in the Truck Side Diesel Dispensers but all the dispenser <br />pans had residuals of diesel. <br />- In 2012, on a weekly basis, an employee would inspect all the Truck Side Diesel Dispensers, pump out <br />any liquid and place the waste into the hazardous waste drum. It appears approximately 1.5 gallons is <br />pumped out weekly. Documentation is made on the UST Recordable Release/ Spill Log. <br />- In 2013, on a monthly basis, a wide range amount of diesel is pumped out of Truck Side Diesel <br />Dispensers. The documentation notes the amount ranges from 0.85L to 51 L. Store Manager is unable <br />decipher the notes on the UST Recordable Release/ Spill Log made by the employee DY. <br />All primary containment must be impervious to the liquid and vapor of the substance contained. <br />Immediately discontinue use of failed piping and have a properly licensed, trained and certified <br />contractor replaced or repair the failed piping under permit and inspection of EHD. If the failed <br />components can't be replaced/ repaired immediately, there is a possibility that the Auto & Truck Side <br />diesel UST system may be red tagged to prevent fuel inputs. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />EHD Inspector: <br />Thuy Tran (209) 468-8257'�►.`�`� <br />Received <br />Title: <br />EHD 23-02-003 Rev 04/19/12 CONTINUATION FORM <br />
