Failed to obtain a permit to repair the UST system. History of all Truck Side alarms were wiped out <br />due to unknown reason in July 2012. On July 20, 2012, Epic Compliance replaced battery. Unable to <br />find work order for the battery replacement. When testing the Truck Side UDC liquid sensors, the liquid <br />senors did not perform positive shutdown on the either of the North and South Diesel turbines. It was <br />later discovered the sensor setup for shutdown was incorrectly programmed. Technician correctly <br />reprogrammed the sensor to perform shutdown for the correct turbine. It was unclear if the Veeder Root <br />had to be cold started and reprogrammed back on July 20, 2012. A permit is required if the monitoring <br />system had to be cold started. <br />Submit to SJC EHD within 30 days (08/12/13): <br />1. one copy of the completed "Return to Compliance Certification" form <br />2. a written corrective actions statement for each of the above violations <br />3. request/ supporting documentation <br />Notes: <br />-Auto Side annual monitoring system certification was completed before noon time. Truck side was not <br />completed until 5:30 that evening.
