Respondent admits the violations described above. <br />SCHEDULE FOR COMPLIANC <br />9. The violations have been corrected. <br />PAYMENTS <br />10. Respondent shall pay the CUPA a total of $52,128.00 of which $50,000 <br />is a penalty and $2128.00 is reimbursement of the CUPA's costs, through the <br />effective date. Respondent shall make an initial payment of $2128.00 within <br />days of the effective date of this consent order. Subsequent payments of <br />$25000.00 shall be due January 23, 2020, and March 23, 2020, for a total of <br />$52,128.00. All additional CUPA costs will be billed at the current hourly rate. <br />Respondent's checks shall be made payable to "San Joaquin County <br />Environmental Health Department," and shall be delivered together with the <br />attached Payment Voucher to: <br />San Joaquin County Environmental Health Department <br />Certified Unified Program CUPA <br />1868 E. Hazelton Avenue <br />Stockton, California 95205 <br />A photocopy of the check shall be sent to the following CUPA personnel <br />assigned to this matter, Michelle Henry, San Joaquin County Environmental <br />Health Department, <br />If Respondent fails to make payments as provided above, <br />Respondent agrees to pay interest at the rate established pursuant to HSC § <br />25360.1 and to pay all costs incurred by the CUPA in pursuing collection <br />including attorney's fees. <br />OTHER PROVISIONS <br />11.
