Left for the site with Fred Ernest (EHD) to verify clean up. Arrived on
site and checked in at the guard station. Met with Derek Smith and clean up crew
with Patriot Environmental. The clean-up crew was in the process of reloading the
container when we arrived and was nearly done—We discussed clean-up with
Derek, who indicated that they applied absorbent to the spilled container
floorboards and containerized the waste. He checked to see if he had a picture for
the final clean-up and indicated that he did not—verified that pavement staining
outside the container was cleaned up using absorbent and that the container,
which had oily staining on top and its sides, was wiped down. Asked Derek if he
was aware of any material spilled outside the container before it was moved, he
indicated that he was not aware of any additional spilling that needed to be
cleaned up. Observed the punctured 300-gallon tote; the puncluture mark was
close to the top of the container. Derek indicated that the coolant from the
punctured tote was transferred to one 250-gallon tote and one 55-gallon
container; the transferred material was loaded back into the container to be
delivered to the destination facility as product. The punctured tote will be cut up
and recycled (per Derek). Observed one black 55-gallon container of generated
waste clean-up debris; was told that all generated debris would be disposed of as
hazardous waste; Derek will send a copy of the initial manifest and the labeled
hazardous waste container shortly.
06/16/2021 Complaint documentation, completed narrative, updated NTA with corrected
MANZO
information and sent an updated NTA to M. Algots. Organized complaint
paperwork.
5105 rpt Page 1 of 4
