Complaint Investigation Summary <br /> Report* 5105 <br />COMPLAINT ID: CO1W8AF1P Site Location: 1000 EAST ROTH ROAD, LATHROP, CA <br />Activity Summary <br />Activity Date Recorded by <br />06/16/2021 2:41 am received a call from M. Naidu (program coordinator) concerning a spill MANZO <br />report. Called General Hospital and obtained the spill information. UP Railroad is <br />reporting a spill of approximately 5-gallons of unknown liquid from a container at <br />1000 Roth Road, Cal OES spill report 21-3191, UP report 2021-06-160178,10. <br />The UP call center number is 888-877-7267. Called the UP contact, Jamin <br />reported at 1:29 am a spill was discovered from container # EMHU-650-154. <br />Patriot Environmental is in route, ETA is unknown at this time. Mike Algots will be <br />the contact for this spill response. Jamin reported that she doesn't have a contact <br />information to someone who is currently onsite. Mike Algots (916) 580-9999. <br />Called Mike Algots, Mike reported that the container carried a mixed load of <br />hazardous materials including flammable liquids. Patriot Environmental is still in <br />route. Reviewed Cal OES report. Generated a complaint record. Sent a text to <br />Mike Algots asking for an update. 4:04 am, called Mike Algots, he reported that <br />the # EMHU-650-154 leaked on the container below it, no spill was observed to <br />have hit the ground. # EMHU-650-154 container was opened and the PID <br />readings were recorded at 5 ppm. At this time Mike is unsure of the spilled <br />chemical. 4:44 am, received a page from the General Hospital. Called the <br />hospital, received an update for this spill. The spill was identified as a 330 gallon <br />tote, which released a 20 gallon of 5NILL coolant. Received UP call center <br />number for this update 888-877-7267. Received a text update from M. Algots, the <br />Patriot will be onsite at 5 or 6 am, Mike indicated that UPS will likely be the <br />responsible party for this spill. Received contact information for Derek Smith with <br />Patriot Environmental, 916-347-8724. 5:15 am, started NTA. 6:28 am, arrived <br />onsite, met with Derek Smith, observed the stained containers, a few drops and <br />minor pooling of dark liquid was noted on the ground below both containers_ <br />Derek indicated that they are waiting for some equipment to arrive to continue <br />with offloading of the containers.
