Complaint investigation Summary Report tt 5/05 <br />COMPLAINT ID: CO1W8AF1P Site Location: 1000 EAST ROTH ROAD, LATHROP, CA <br />Activity Date <br />Activity Summary <br />Recorded by <br />06/16/2021 Traveled with Elena M. to 1000 Roth Rd in Lathrop to observe the affected area <br />from a Coolant spill due to a broken 300 gallon tote in a rail car (EMHU 650154). <br />ERNEST <br />Received permission to visit cleanup site, from the railway security office. <br />Container was mostly loaded when we arrived on site. <br />An approximately 300 gallon Tote was emptied and remove from the car. A 55 <br />gallon drum of waste absorbent was in the work area. <br />Damaged tote has a label that indicated the material is AFM-COOL- A-10 <br />SYNTHETIC COOLANT. <br />Tote had a crack in one of the top corners of the container. <br />Left area due to cleanup crew estimating cleanup will take over 3 hours. <br />Coolant observed on site was labeled as an irritant. <br />Site estimates approximately 20 gallons was lost during transit, and on site. <br />Filled out a spill report, prop 65, created a photo log. <br />Photos sent to Elena M. <br />Received clean up photos, along with hazardous waste manifest for the cleanup <br />site. <br />Derek from patriot environmental provided photos of the clean up. <br />Emailed jlwomack@ups.com to verify responsible party information. <br />06/17/2091 Received a text message from M. Algots with UPS responsible party contact <br />information, jlwomack@ups.com. Prop 65 preparation, provided to G.
