MURO <br />03/13/2020 Completed Emergency Response Record, Site map, Notification of Hazardous MURO <br />Waste Discharge Prop 65. Took Prop 65 to lead senior for review. <br />03/13/2020 Completed narrative for spill incident. MURO <br />03/26/2020 Called Carlos Sanchez, American Integrated Services, Inc Hazmat Environmental MURO <br />Manager, 707-210-5529 to inquire whether the spill was stopped at the time of his <br />arrival. He indicated that it was stopped by BNSF mechanics at the time of his <br />arrival. <br />05/06/2020 Emailed Jeffrey Hankins, BNSF, jeffrey.hankins2@bnsf.com to follow up with <br />analytical testing results and waste disposal. <br />MURO <br />05/07/2020 Received response to my email from Jeff Hankins, BNSF. He indicated that there <br />was no lab analysis done on the waste generated from cleanup on 3-12-20 <br />because it was characterized and managed as Non-RCRA Hazardous Waste <br />MURO <br />Solid. The waste has already been picked up for disposal. I replied with request for <br />a copy of the hazardous waste manifest. <br />Jeff Hankins emailed a copy of the uniform hazardous waste manifest, <br />021324617JJK with ship date 4-2-20 for the disposal of one 55-gallon drum, 200 <br />pounds of Non-RCRA Hazardous Waste, Solid ( soil/ballast and diesel). <br />01/04/2021 Reviewed file in preparation for abatement. MURO <br />05/12/2021 Received a call transfer from accounting, spoke with Jeffrey Hankins concerning <br />complaint billing, emailed narrative log (per his request) to <br />jeffrey.hankins2@bnsf.com. <br />MANZO <br />05/27/2021 File preparation for abate. MURO <br />09/07/2021 Compiled complaint file in order; added sticky note of items still required to print. MURO <br />-log 2 <br />-emails <br />-5105 <br />-ER01 <br />-initial photo log <br />-final manifest <br />09/22/2021 MURO <br />09/24/2021 Reviewed complaint to begin abatement procedure. Printed all related documents <br />associated with this complaint. Compiled documents into proper abatement <br />sequence.
