Complaint Investigation Summary Report #: 5105 COMPLAINT ID: C00051745 Site Location: 720 S B ST Activity Summary Activity Date Recorded by 03/12/2020 2:44 PM Received page from EHD front desk. I went to front desk to receive the MURO information from Genesis Martinez. Complaint information is: Approximately five gallons of diesel fuel spilled. OES: 20-1456. 2:52 PM I called complainant, Jeff Hankins, BNSF Hazmat Manager, 505-218-3582. Jeff Hankins confirmed the 5 gallon diesel spill. He indicated it was contained. I asked him how contained, he did not have that information. I asked him when it happened. He did not have that information. He said the spill was due to a maintenance issue with the locomotive. BNSF is the responsible party. No water ways or drains were impacted. Contractor American Integrated Services (AIS) was en route from Fairfield, CA with ETA to be "any minute now." BNSF Mechanical Foreman is on site, his name and contact number is: Michael Larson (209) 401-9142. It was determined EHD will respond to spill incident because it is not know if the leak has stopped. 3:04 PM Arrived at BNSF with Elena Manzo, Lead Senior REHS. Called Michael Larson. Michael was away in Fresno, but directed my to the train master, Kevin. Discussed spill location with Kevin. The spill is on the Eastern side of the yard and can be accessed by driving East on Martin Luther King Blvd then turning left on Diamond St. Kevin was unable to accompany us, as he was unable to leave his post. However, he showed us the locomotive and spill location on their camera. We asked Kevin if he knew if the leak was stopped. He was not exactly certain, but knew that when these incidents happen, the protocol is to shut off the fuel line. Called Michael Larson, left message to ask if the leak had been stopped. 3:26 PM Called AIS, (707) 437-2200, to find the Hazmat contact on route to BNSF spill location. I was given Hazmat Environmental Manager name and phone number: Carlos Sanchez, (707) 210-5529. 3:29 PM Called Carlos Sanchez and was given a five minute ETA. Arrived at the Diamond St. spill area. Observed a red, green, and white Ferromex Locomotive #4644 with absorbent pads underneath the locomotive on the North, South, and East sides. Called Jeffrey Hankins for corporate mailing address and email address. BNSF 2650 Lou Menk Drive Fort Worth, TX 76131 email: jeffrey.hankins2@bnsf.com Carlos Sanchez (AIS) arrived. He was on the phone and walking around the locomotive. I generated a Notice to Abate for BNSF to stop spill and disposal records. Emailed to Jeff Hankins at: jeffrey.hankins2@bnsf.com Received call from Michael Larson and was told the diesel was released from a fuel line rupture. He did not know if they stopped the leak at the time BNSF applied the absorbent pads under the locomotive. 4:22 PM Left site. 4:28 PM Arrived at EHD. 03/12/2020 Discussed spill information with Lydia Baker, REHS who is on standby after hours.
